According to Cancer Research UK, over 52,000 men are diagnosed with the disease each year and there are more than 12,000 deaths. Over three-quarters (78%) of men diagnosed with prostate cancer survive for over 10 years, but this proportion has barely changed over the past decade in the UK.</p>&#13; &#13; <p>Testing for prostate cancer involves a blood test that looks for a protein known as a prostate-specific antigen (PSA) that is made only by the prostate gland; however, it is not always accurate. According to the NHS website, around three in four men with a raised PSA level will not have cancer. Further tests, such as tissue biopsies or MRI scans, are therefore required to confirm a diagnosis.</p>&#13; &#13; <p>Professor Antonis Antoniou from the Department of Public Health and Primary Care at the 探花直播 of Cambridge said: 鈥淧rostate cancer is the most common cancer in men in the UK, but population-wide screening based on PSA isn鈥檛 an option: these tests are often falsely positive, which means that many men would then be biopsied unnecessarily. Also, many prostate tumours identified by PSA tests are slow-growing and would not have been life-threatening. 探花直播treatment of these tumours may do more harm than good.</p>&#13; &#13; <p>鈥淲hat we need is a way of identifying those men who are at greatest risk, allowing us to target screening and diagnostic tests where they are most needed, while also reducing the harms for those men who have low risk of the disease. This is what CanRisk-Prostate aims to do. For the first time, it combines information on the genetic makeup and prostate cancer family history, the main risk factors for the disease, to provide personalised cancer risks.鈥</p>&#13; &#13; <p>Prostate cancer is one of the most genetically determined of common cancers. Inherited faulty versions of the BRCA2, HOXB13 and possibly BRCA1 genes are associated with moderate-to-high risk of prostate cancer, though such faults are rare in the population. In addition, there are several hundred more common genetic variants that each confer a lower risk, but in aggregate they act like 鈥榲olume control鈥 that moderate or increase the prostate cancer risk.</p>&#13; &#13; <p>Writing in the <em>Journal of Clinical Oncology</em>, the researchers 鈥 supported by Cancer Research UK 鈥 describe the development of the first comprehensive prostate cancer model using genetic and cancer family history data from almost 17,000 families affected by prostate cancer. It uses data on rare genetic faults in moderate-to-high-risk genes and a risk score based on 268 common low-risk variants, together with detailed cancer family history, to predict the future risks.</p>&#13; &#13; <p>One in six men (16%) will develop prostate cancer by the time they are 85 years old. Using the model, the team found that the predicted risk was higher for men who had a father diagnosed with prostate cancer 鈥 27% if the father was diagnosed at an older age (80 years) but as high as 42% if the father was diagnosed at a young age (50 years).</p>&#13; &#13; <p> 探花直播risks were considerably higher for men with genetic faults. For example, 54% of men who carry an alteration in the BRCA2 gene would develop prostate cancer 鈥 however, among men with BRCA2 gene faults, the risks were substantially lower if they also had a small number of the low-risk variants, but much higher if they also had a large number of the low-risk variants.</p>&#13; &#13; <p>In practice, say the researchers, clinicians will be able to use any combination of cancer family history, rare and common genetic variants to provide a personalised risk.</p>&#13; &#13; <p>To validate their model, the team ran the risk model on an independent cohort of over 170,000 men recruited to UK Biobank, a biomedical database and research resource containing anonymised genetic, lifestyle and health information from half a million UK participants. All of these men were free from prostate cancer when they were recruited to the study, but more than 7,600 developed prostate cancer within the subsequent ten years.</p>&#13; &#13; <p>When validating their model, the researchers found that 86% of the UK Biobank participants who developed cancer were in the half of men with the highest predicted risks, which suggests that it may be possible to target screening and diagnostic tests to the subgroup of the population at highest risk, among whom the majority of the cancers will occur. 聽</p>&#13; &#13; <p>Dr Tommy Nyberg from the MRC Biostatistics Unit at Cambridge said: 鈥淲e鈥檝e created the most comprehensive tool to date for predicting a man鈥檚 risk of developing prostate cancer. We hope this will help clinicians and genetic counsellors assess their clients鈥 risk and provide the appropriate follow-up.</p>&#13; &#13; <p>鈥淥ver the next 12 months, we aim to build this tool into the widely used CanRisk tool, which will facilitate the risk-based clinical management of men seen in family cancer clinics and enable risk-adapted early detection approaches to the population at large.鈥</p>&#13; &#13; <p>Professor Ros Eeles from 探花直播Institute of Cancer Research, London and co-author on the study said: 鈥淭his is an important step forward as it will enable clinicians to have conversations with men about their individual risk of prostate cancer based on the most accurate computer model to date. According to Cancer Research UK, over 52,000 men are diagnosed with prostate cancer each year and there are more than 12,000 deaths.</p> <p>Over three-quarters (78%) of men diagnosed with the disease survive for over ten years, but this proportion has barely changed over the past decade in the UK, largely because the disease is detected at a relatively late stage. In England, for example, nearly half of all prostate cancers are picked up at stage three of four (stage four being the latest stage).</p> <p>Despite no evidence of a link between urinary symptoms and prostate cancer, national guidelines, health advice and public health campaigns continue to promote this link. In a review published today in <em>BMC Medicine</em>, Cambridge researchers argue that not only is this unhelpful, but it may even deter men from coming forward for early testing and detection of a potentially treatable cancer.</p> <p>鈥淲hen most people think of the symptoms of prostate cancer, they think of problems with peeing or needing to pee more frequently, particularly during the night,鈥 said Vincent Gnanapragasam, Professor of Urology at the 探花直播 of Cambridge and an Honorary Consultant Urologist at Addenbrooke鈥檚 Hospital, Cambridge. 鈥淭his misperception has lasted for decades, despite very little evidence, and it鈥檚 potentially preventing us picking up cases at an early stage.鈥</p> <p>Prostate enlargement can cause the urinary problems often included in public health messaging, but evidence suggests that this is rarely due to malignant prostate tumours. Rather, research suggests that the prostate is smaller in cases of prostate cancer.聽 A recent study 鈥 the UK PROTECT trial 鈥 even went as far as to say that a lack of urinary symptoms may in fact be an indicator of a higher likelihood of cancer.</p> <p>Screening programmes are one way that cancers are often detected at an early stage, but in the case of prostate cancer, some argue that such programmes risk overwhelming health services and leading to men being treated for relatively benign disease.</p> <p>Testing for prostate cancer involves a blood test that looks for a protein known as a prostate-specific antigen (PSA) that is made only by the prostate gland; however, it is not always accurate. PSA density is significantly more accurate than PSA alone in predicting a positive biopsy and is used in everyday clinical practice.</p> <p> 探花直播researchers point to evidence that there is a misconception that prostate cancer is always symptomatic: a previous study found that 86% of the public associated prostate cancer with symptoms, but only 1% were aware that it could be asymptomatic.</p> <p>鈥淲e urgently need to recognise that the information currently given to the public risks giving men a false sense of security if they don鈥檛 have any urinary symptoms,鈥 said Professor Gnanapragasam.</p> <p>鈥淲e need to emphasise that prostate cancer can be a silent or asymptomatic disease, particularly in its curable stages. Waiting out for urinary symptoms may mean missing opportunities to catch the disease when it鈥檚 treatable.</p> <p>鈥淢en shouldn鈥檛 be afraid to speak to their GP about getting tested, and about the value of a PSA test, especially if they have a history of prostate cancer in their family or have other risk factors such as being of Black or mixed Black ethnicity.鈥</p> <p> 探花直播researchers say they are not advocating for an immediate screening programme, and acknowledge that changes in messaging could mean more men approaching their GPs for a PSA test, potentially resulting in unnecessary investigations and treatment. However, they argue that there are ways to reduce the risk of this happening. These include the use of algorithms to assess an individual鈥檚 risk and whether they need to be referred to a specialist, and for those who are referred, MRI scans could help rule out 鈥榠ndolent鈥 (mild) disease or negative findings, reducing the risks of an unnecessary biopsy.</p> <p>鈥淲e鈥檙e calling on organisations such as the NHS, as well as patient charities and the media, to review the current public messaging,鈥 said Professor Gnanapragasam.</p> <p>鈥淚f men were aware that just because they have no symptoms doesn鈥檛 necessarily mean they are cancer free, then more might take up offers for tests. However, these studies had small sample sizes, resulting in imprecise estimates of cancer risk. Being able to estimate the risks accurately is important for informing cancer prevention and screening strategies and providing genetic counselling to those at greatest risk. BRCA mutations are uncommon, <a href="https://www.nhs.uk/conditions/predictive-genetic-tests-cancer/">affecting around 1 in 300-400 people in the population</a>.</p>&#13; &#13; <p>To further investigate these risk estimates, a team led by researchers at the 探花直播 of Cambridge, funded by Cancer Research UK, analysed data from almost 3,200 families with one or more members with the BRCA1 mutation and almost 2,200 families with members carrying the BRCA2 mutation. 探花直播families had all been recruited to the Consortium of Investigators of Modi铿乪rs of BRCA1/2. 探花直播researchers examined the associations with 22 primary cancers.</p>&#13; &#13; <p>From the data, the researchers estimated that men who carry a BRCA2 mutation have a 27% risk of developing prostate cancer by the time they are 80 years old, more than double the rate compared to non-carriers. BRCA1 mutations were not associated with an increase in prostate cancer risk.</p>&#13; &#13; <p>Carrying a defective copy of either BRCA1 or BRCA2 more than doubled an individual鈥檚 risk of pancreatic cancer to 2.5-3% by age 80.</p>&#13; &#13; <p> 探花直播mutations were also found to increase the risk of stomach cancer, though the researchers caution that because of the rarity of this form of cancer, the number of patients in their datasets was small.</p>&#13; &#13; <p>Mutations in both genes significantly increased the risk of breast cancer in men, though the disease is still very rare, <a href="https://www.cancerresearchuk.org/about-cancer/breast-cancer/types/male-breast-cancer">accounting for less than 1% of all male cancer cases in the UK</a>. While a BRCA1 mutation increased a man鈥檚 risk of developing breast cancer more than four-fold to 0.4% by age 80, a BRCA2 mutation increased this risk by 44 times to 3.8% by age 80. It is estimated that 38 out of 1,000 male carriers of the BRCA2 mutation will develop breast cancer by age 80.</p>&#13; &#13; <p> 探花直播researchers were unable to find compelling evidence that mutations were linked to increased risk of some other cancers which were previously thought to be linked to faulty BRCA genes, such as melanoma.</p>&#13; &#13; <p>Cancer Research UK says that people who are worried about their risk of cancer should talk to their GP. GPs can refer patients to a genetics clinic if they think someone has a strong family history and might be at an increased risk.</p>&#13; &#13; <p>Professor Antonis Antoniou from the Department of Public Health and Primary Care at the 探花直播 of Cambridge, who led the research, said: 鈥淭hese large datasets of patients have allowed us to estimate with much greater accuracy the extent to which faulty BRCA1 and BRCA2 genes increase the risk of several cancers. We鈥檝e known for some time that they鈥檙e linked to breast and ovarian cancer, but there鈥檚 been uncertainty about other cancers.鈥</p>&#13; &#13; <p>Professor Marc Tischkowitz from the Department of Medical Genetics at the 探花直播 of Cambridge added: 鈥 探花直播link between BRCA2 and prostate cancer and pancreatic cancer is now much clearer, thanks to the data we鈥檝e analysed. We have also identified a potential link with stomach cancer, but this is based on small numbers and needs further study. Our data suggests that there is no strong link between BRCA2 and melanoma, which may provide greater clarity to BRCA2 gene carriers.</p>&#13; &#13; <p>鈥淥verall, the results will add to our knowledge on optimising cancer screening and early detection strategies for people who are known to carry these faulty genes.鈥</p>&#13; &#13; <p>Michelle Mitchell, Chief Executive of Cancer Research UK, said: 鈥淥ur scientists helped to discover BRCA over 25 years ago and established that faults in these genes increase breast cancer risk. It brings together the latest evidence and mathematical models to give a personalised prognosis, which the researchers say will empower patients as they discuss treatment options with their consultant.</p>&#13; &#13; <p><a href="https://www.cancerresearchuk.org/health-professional/cancer-statistics/statistics-by-cancer-type/prostate-cancer">According to Cancer Research UK</a>, there were 47,151 new cases of prostate cancer in 2015. Progression of the disease, which usually presents in later life, is very variable: in most cases, the disease progresses slowly and is not fatal. It is often said that more men die with prostate cancer than from it. However, it is still the case that in a significant number of men, the tumour will metastasise and spread to other organs, threatening their health.</p>&#13; &#13; <p>When a patient is diagnosed with prostate cancer, they are currently classified as low, intermediate or high risk. Depending on the patient鈥檚 risk group, clinicians will recommend either an 鈥榓ctive monitoring鈥 approach or treatment. Treatment options include radiotherapy or surgery and can have potentially significant side-effects, including erectile dysfunction and urinary incontinence.</p>&#13; &#13; <p>However, evidence suggests that these classifications, which are in the current guidelines provided by the National Institute for Health and Care Excellence (NICE), are only 60-70% accurate. This means that many men may elect for treatment when it is not necessary. In fact, <a href="https://www.nejm.org/doi/full/10.1056/NEJMoa1606220">a recent study</a> carried out in the UK showed that for early prostate cancer (low and intermediate risk), treatment is no more beneficial in terms of ten year survival compared to no treatment.</p>&#13; &#13; <p>Cambridge researchers have already shown that it is possible to improve the accuracy of the NICE-endorsed model to more than 80% by <a href="https://pubmed.ncbi.nlm.nih.gov/27483464/">stratifying patients into five rather than three groups</a>. Their next challenge was to use this information to give a more individual prediction of outcome to patients at no extra cost. 探花直播result is <a href="https://prostate.predict.cam/">PREDICT Prostate</a>.</p>&#13; &#13; <p>PREDICT Prostate takes routinely available information including PSA test results, the cancer grade and stage, the proportion of biopsies with cancerous cells, and details about the patient including his age and other illnesses. It then gives a 10-15 year survival estimate. Importantly, the tool also estimates how his chance of survival differs depending on whether he opts for monitoring or treatment, providing context of the likelihood of success of treatment and risk of side effects.</p>&#13; &#13; <p>鈥淎s far as we are aware, this is the first personalised tool to give an overall survival estimate for men following a prostate cancer diagnosis,鈥 says first author Dr David Thurtle, Academic Clinical Fellow in Urology at the 探花直播 of Cambridge and Addenbrooke鈥檚 Hospital, which is part of Cambridge 探花直播 Hospital NHS Foundation Trust (CUH).</p>&#13; &#13; <p>鈥淧REDICT Prostate is designed for men who are considering whether to choose to monitor or to opt for treatment. This is the choice that faces nearly half of all men who are diagnosed with prostate cancer. We hope it will provide a more accurate and objective estimate to help men reach an informed decision in discussion with their consultant.鈥</p>&#13; &#13; <p> 探花直播research was led by Dr Vincent Gnanapragasam, 探花直播 Lecturer and Honorary Consultant at CUH, and undertaken by Dr Thurtle, both of the Academic Urology Group in Cambridge, and in collaboration with Professor Paul Pharoah of the Department of Cancer Epidemiology.</p>&#13; &#13; <p>鈥淲e believe this tool could significantly reduce the number of unnecessary 鈥 and potentially harmful 鈥 treatments that patients receive and save the NHS millions every year,鈥 says Dr Gnanapragasam.</p>&#13; &#13; <p>鈥淭his isn鈥檛 about rationing treatments 鈥 it鈥檚 about empowering patients and their clinicians to make decisions based on better evidence. In some cases, treatment will be the right option, but in many others, patients will want to weigh up the treatment benefits versus the risks of side effects. It will also show men who do need treatment a realistic estimate of their survival after treatment.鈥</p>&#13; &#13; <p>Data from the National Prostate Cancer Audit has shown that rates of treatment for low risk prostate cancer vary across different hospitals between 2-25%. 鈥楻adical鈥 treatment 鈥 surgery or radiotherapy, for example 鈥 costs on average around 拢7,000 per patient and treating these men unnecessarily wastes considerable resources as well as causing significant side-effects.</p>&#13; &#13; <p>Dr Thurtle and Dr Gnanapragasam have since carried out a randomised study of almost 200 prostate cancer specialists in which they gave some clinicians access to the tool and a series of patient vignettes, while others received the vignettes only. In most cases, the clinician overestimated the risk of the patient dying from the cancer, compared to the estimate given by PREDICT, going on to recommended treatment in many cases and overestimate how successful this treatment would be. When given access to the tool, the clinicians were less likely to recommend treatment in good prognosis cancers.</p>&#13; &#13; <p>Dr Gnanapragasam says that the development of PREDICT Prostate has only been possible because of the intactness of records available through Public Health England 鈥 the tool was developed using data from over 10,000 UK men recorded in the East of England. This regional registry, he says, is one of the highest quality and most comprehensive data sets available both in the UK and internationally. 探花直播data was then validated externally in a sample of 2,500 prostate cancer patients in Singapore. 探花直播web tool was developed in collaboration with the Winton Centre for Risk and Evidence Communication</p>&#13; &#13; <p> 探花直播researchers caution that the tool is strongly recommended for use only in consultation with a clinician. Patients who undergo this procedure are at risk of urinary infections or sepsis as the needle has to pass through the bowel wall to reach the prostate.聽 Around 30-40,000 prostate biopsies are done every year using this method in the UK alone.</p>&#13; &#13; <p> 探花直播CamProbe, invented and developed in Cambridge, has been designed so the biopsies can be taken more safely through the skin under the scrotum (transperineal) and avoid the bowel.</p>&#13; &#13; <p>鈥 探花直播design of the CamProbe is a needle within a needle and allows us to collect tissue from the prostate through a more sterile part of the body,鈥 says Dr Gnanapragasam, who co-leads the Urological Malignancies Programme at the CRUK Cambridge Centre.</p>&#13; &#13; <p>鈥淢ost importantly it can be done under local anaesthetic in the out-patient department. Previously this kind of approach was only possible if a general anaesthetic was used with very significant additional costs.鈥</p>&#13; &#13; <p> 探花直播trial for the CamProbe is now underway using funding from the National Institute for Health Research (NIHR). It will run for a year at several hospitals around the UK including at the Cambridge Clinical Research Centre. With its mix of medicine and surgery, utilisation of technology such as lasers and robots, and treatment of conditions that have profound impacts on patients鈥 quality and length of life 鈥淯rology has it all!鈥 he says.</p>&#13; &#13; <p>Since starting an Academic Clinical Fellowship in Cambridge in 2014, he has worked on a range of clinical prostate cancer related topics, collaborating with radiologists, engineers and epidemiologists amongst many others. 鈥淚 love the daily interaction and satisfaction of clinical medicine and have always sought out research projects that are 鈥榗lose to the coal-face鈥 of clinical work.鈥</p>&#13; &#13; <p>David鈥檚 research sets out to inform both patients and doctors about the long term survival outcomes for men diagnosed with localised prostate cancer.</p>&#13; &#13; <p>鈥淧rostate cancer has many different guises 鈥 some cases are indolent and may never impact upon a patient鈥檚 length of life, while others can rapidly metastasise causing significant problems and shortening life. It is the fourth most commonly diagnosed cancer in UK women, with around 9,000 new cases being diagnosed each year.<br /><br />&#13; Researchers at the 探花直播 of Cambridge, Oxford 探花直播 and QIMR Berghofer Medical Research Institute in Brisbane studied the DNA of over 7,000 women with endometrial cancer and 37,000 women without cancer to identify genetic variants that affected a woman鈥檚 risk of developing the disease. 探花直播results are published today in the journal Nature Genetics.<br /><br />&#13; Dr Deborah Thompson from the Department of Public Health and Primary Care at the 探花直播 of Cambridge said: 鈥淥ur findings help us to paint a clearer picture of the genetic causes of endometrial cancer in women, particularly where there no strong family history of cancer. Prior to this study, we only knew of four regions of the genome in which a common genetic variant increases a woman鈥檚 risk of endometrial cancer.<br /><br />&#13; 鈥淚n this study we have identified another five regions, bringing the total to nine.
