Overall, females were more successful than males (37.0% versus 29.1%).</p>&#13; &#13; <p> 探花直播researchers found clear evidence that certain specialities were more attractive to females or to males. Of note, surgical specialities and radiology had the highest proportion of male applicants (65.3% and 64.3% respectively), while obstetrics and gynaecology and public health had the highest proportion of female applicants (72.4% and 67.2% respectively).</p>&#13; &#13; <p>Senior author Professor Sharon Peacock, from the Department of Medicine at the 探花直播 of Cambridge, said: 鈥 探花直播success by female applicants in many specialties is a positive step towards gender balance, and perhaps reflects existing efforts to address disparities. But the skew in applications and subsequent recruitment by gender, particularly amongst surgical specialities, is concerning.鈥</p>&#13; &#13; <p>Gender disparities are known to have knock-on effects. For example, a lack of female representation contributes towards a male dominated culture, which can then result in fewer female role models to inspire and encourage aspiring female doctors.</p>&#13; &#13; <p> 探花直播researchers say there are several reasons for these disparities. In surgical specialities, for example, a male-dominated workplace culture, bullying and harassment, few female role models, and career inflexibility, have been suggested as factors that deter females from applying. Female surgeons have reported quality of life and fewer unsocial hours as explanations of why women prefer other clinical specialities, in addition to the fear that working less-than-full-time or taking career breaks is perceived negatively.</p>&#13; &#13; <p>Approximately half (50.2%) of the applicants were non-UK graduates. 探花直播overall success rate of UK graduates was 44.5%, compared with 22.8% for non-UK graduates.</p>&#13; &#13; <p>When it came to minority ethnic groups, after adjusting for country of graduation, applicants from eleven out of fifteen groups (73.3%) were significantly less likely to be successful compared to White British. Those who fared worst were those of Mixed White and Black African ethnicity, who were only half as likely (52%) to be successful as White British applicants.</p>&#13; &#13; <p>Dr Dinesh Aggarwal, the study鈥檚 first author, also from the Department of Medicine, said: 鈥 探花直播data suggests there鈥檚 a need to review recruitment policies and processes from a diversity and inclusion perspective. But the issues extend beyond recruitment 鈥 doctors from minority ethnic groups can struggle to progress within the NHS and report disproportionately high levels of discrimination from colleagues.</p>&#13; &#13; <p>鈥淢ore than four in ten of the medical and dental workforce in NHS trusts and clinical commissioning groups in England are from a minority ethnic group, and ensuring that they are able to work within an inclusive environment, that allows them to thrive and progress, should be a priority.鈥</p>&#13; &#13; <p>Although only a very small proportion of successful applicants (1.4%) declared a disability, they were more likely to be successful (38.6% compared with 32.8% of non-disabled applicants). However, there were no disabled applicants to 22.4% of the specialities, and for a further 36.2% of specialities, no disabled applicants were accepted.</p>&#13; &#13; <p>Dr Dinesh Aggarwal added: 鈥淚t鈥檚 encouraging to see a high proportion of acceptances among individuals disclosing a disability. 探花直播NHS needs to ensure that application and recruitment processes are accessible and open to adjustments for all disabilities, eliminate any fear of discrimination, and provide assurance that all NHS workplaces will accommodate reasonable adjustments to ensure that disabled doctors can carry out their work. But as hospitals have seen their capacity overwhelmed, they have been forced to manage many of their COVID-19 patients in repurposed 鈥榮urge鈥 wards, which often lack the ability to change the air with a high frequency. While the use of appropriate personal protective equipment (PPE) protects staff and patients significantly reduces the risk of transmission, there are still reports of patient-to-healthcare worker transmission of the virus, potentially through the inhalation of viral particles.</p> <p>A team at the 探花直播 of Cambridge and Cambridge 探花直播 Hospitals (CUH) NHS Foundation Trust investigated whether portable air filtration/UV sterilisation devices could reduce airborne SARS- CoV-2 in general wards that had been repurposed as a COVID ward and a COVID Intensive Care Unit (ICU). 探花直播results are published in Clinical Infectious Diseases.</p> <p>Dr Vilas Navapurkar, a Consultant in Intensive Care Medicine at CUH, who led the study, said: 鈥淩educing airborne transmission of the coronavirus is extremely important for the safety of both patients and staff. Effective PPE has made a huge difference, but anything we can do to reduce the risk further is important.鈥</p> <p>鈥淏ecause of the numbers of patients being admitted with COVID-19, hospitals have had to use wards not designed for managing respiratory infections. During an intensely busy time, we were able to pull together a team from across the hospital and 探花直播 to test whether portable air filtration devices, which are relatively inexpensive, might remove airborne SARS-CoV-2 and make these wards safer.鈥</p> <p> 探花直播team performed their study in two repurposed COVID-19 units in Addenbrooke鈥檚 Hospital. One area was a surge ward managing patients who required simple oxygen treatment or no respiratory support; the second was a surge ICU managing patients who required ventilation either through non-invasive mask ventilation or invasive respiratory support, such as involving the use of an invasive tube and tracheostomy.</p> <p> 探花直播team installed a High Efficiency Particulate Air (HEPA) air filter/UV steriliser. HEPA filters are made up of thousands of fibres knitted together to form a material that filters out particles above a certain size. 探花直播machines were placed in fixed positions and operated continuously for seven days, filtering the full volume of air in each room between five and ten times per hour.</p> <p>In the surge ward, during the first week prior to the air filter being activated, the researchers were able to detect SARS-CoV-2 on all sampling days. Once the air filter was switched on and run continuously, the team were unable to detect SARS-CoV-2 on any of the five testing days. They then switched off the machine and repeated the sampling 鈥 once again, they were able to detect SARS-CoV-2 on three of the five sampling days.</p> <p>On the ICU, the team found limited evidence of airborne SARS-CoV-2 in the weeks when the machine was switched off and traces of the virus on one sampling day when the machine was active.</p> <p>Additionally, the air filters significantly reduced levels of bacterial, fungal and other viral bioaerosols on the both the surge ward and the ICU, highlighting an added benefit of the system.聽</p> <p><img alt="" src="/sites/www.cam.ac.uk/files/inner-images/vilas_2edit.jpg" style="width: 440px; height: 600px; float: left; margin: 5px;" /></p> <p>First author Dr Andrew Conway Morris, from the Department of Medicine at the 探花直播 of Cambridge, said: 鈥淲e were really surprised by quite how effect air filters were at removing airborne SARS-CoV-2 on the wards. Although it was only a small study, it highlights their potential to improve the safety of wards, particularly in areas not designed for managing highly infectious diseases such as COVID-19.鈥</p> <p>Crucially, the research team developed a robust technique for assessing the quality of air, involving placing air samplers at various points in the room and then testing the samples using PCR assays similar those used in the 鈥榞old standard鈥 COVID-19 tests.</p> <p>Professor Stephen Baker, from the Cambridge Institute of Therapeutic Immunology and Infectious Disease at the 探花直播 of Cambridge, said: 鈥淐leaner air will reduce the risk of airborne disease transmission, but it鈥檚 unlikely to be the case that just installing an air filter will be enough to guarantee the air is clean enough. Every room and every situation will be different. A key part of our work has been developing a robust way of measuring air quality.鈥</p> <p>Dr Navapurkar added: 鈥淲e鈥檙e all familiar with the idea of having standards for clean water and of hygiene standards for food. Minor differences in facial features, such as the amount of fat under the skin, make significant differences in how well a mask fits.</p> <p> 探花直播<a href="https://journals.plos.org/plosone/article?id=10.1371/journal.pone.0245688">results</a>, published in the journal <em>PLoS ONE</em>, also suggest that the fit-check routine used in many healthcare settings has high failure rates, as minor leaks may be difficult or impossible to detect by the wearer. While the sample size was small, the researchers hope their findings will help develop new fit tests that are quick and reliable, in the case of future public health emergencies. 探花直播current study only evaluated the impact of fit on the wearer of the mask 鈥 the team will evaluate how fit impacts the protection of others in future research.</p> <p> 探花直播COVID-19 pandemic has made well-fitting face masks a vital piece of protective equipment for healthcare workers and civilians. While the importance of wearing face masks in slowing the spread of the virus has been demonstrated, there remains a lack of understanding about the role that good fit plays in ensuring their effectiveness.</p> <p>鈥淲e know that unless there is a good seal between the mask and the wearer鈥檚 face, many aerosols and droplets will leak through the top and sides of the mask, as many people who wear glasses will be well aware of,鈥 said Eugenia O鈥橩elly from Cambridge鈥檚 Department of Engineering, the paper鈥檚 first author. 鈥淲e wanted to quantitatively evaluate the level of fit offered by various types of masks, and most importantly, assess the accuracy of implementing fit-checks by comparing fit-check results to quantitative fit testing results.鈥</p> <p>For the study, seven participants first evaluated N95 and KN95 masks by performing a fit check, according to NHS guidelines. Participants then underwent quantitative fit testing 鈥 which uses a particle counter to measure the concentration of particles inside and outside the mask 鈥 while wearing N95 and KN95 masks, surgical masks, and fabric masks. 探花直播results assessed the protection to the mask wearer, which is important in clinical settings.</p> <p>N95 masks 鈥 which are a similar standard to the FFP3 masks available in the UK and the rest of Europe 鈥 offered higher degrees of protection than the other categories of masks tested; however, most N95 masks failed to fit the participants adequately.</p> <p>In their study, the researchers found that when fitted properly, N95 masks filtered more than 95% of airborne particles, offering superior protection. However, in some cases, poorly-fitted N95 masks were only comparable with surgical or cloth masks.</p> <p>鈥淚t鈥檚 not enough to assume that any single N95 model will fit the majority of a population,鈥 said O鈥橩elly. 鈥 探花直播most widely-fitting mask we looked at, the 8511 N95, fit only three out of the seven participants in our study.鈥</p> <p>One observation the researchers made during their study was the width of the flange of the mask - the area of the material which comes in contact with the skin 鈥 may be a critical feature to fit. Masks which fit the greatest number of participants tended to have wider, more flexible flanges around the border.</p> <p>In addition, small facial differences were observed to have a significant impact on quantitative fit. 鈥淔itting the face perfectly is a difficult technical challenge and, as our research showed, small differences such as a centimetre wider nose or slightly fuller cheeks can make or break the fit of a mask,鈥 said O鈥橩elly.</p> <p>Self-performed fit-checks are attractive because they save on time and resources, and are often the only method of fit testing available. However, this study, and studies of fit-check systems in other countries, indicate that such fit-check systems are not reliable.</p> <p> 探花直播researchers hope that their results will be of use for those who are working on new technologies and programmes to assess fit, so that healthcare and other frontline workers are adequately protected in the case of any future pandemics. However, these patients are susceptible to further infections from bacteria and fungi that they may acquire while in hospital 鈥 so called 鈥榲entilator-associated pneumonia鈥.</p> <p>Now, a team of scientists and doctors at the 探花直播 of Cambridge and Cambridge 探花直播 Hospitals NHS Foundation Trust, led by Professor Gordon Dougan, Dr Vilas Navapurkar and Dr Andrew Conway Morris, have developed a simple DNA test to quickly identify these infections and target antibiotic treatment as needed.</p> <p> 探花直播test, developed at Addenbrooke鈥檚 hospital in collaboration with Public Health England, gives doctors the information they need to start treatment within hours rather than days, fine-tuning treatment as required and reducing the inappropriate use of antibiotics. This approach, based on higher throughput DNA testing, is being rolled out at Cambridge 探花直播 Hospitals and offers a route towards better treatments for infection more generally. 探花直播<a href="https://ccforum.biomedcentral.com/articles/10.1186/s13054-021-03460-5">results</a> are reported in the journal <em>Critical Care</em>.</p> <p>Patients who need mechanical ventilation are at significant risk of developing secondary pneumonia while they are in intensive care. These infections are often caused by antibiotic-resistant bacteria, and are hard to diagnose and need targeted treatment.</p> <p>鈥淓arly on in the pandemic we noticed that COVID-19 patients appeared to be particularly at risk of developing secondary pneumonia, and started using a rapid diagnostic test that we had developed for just such a situation,鈥 said co-author Dr Andrew Conway Morris from Cambridge鈥檚 Department of Medicine and an intensive care consultant. 鈥淯sing this test, we found that patients with COVID-19 were twice as likely to develop secondary pneumonia as other patients in the same intensive care unit.鈥</p> <p>COVID-19 patients are thought to be at increased risk of infection for several reasons. Due to the amount of lung damage, these severe COVID-19 cases tend to spend more time on a ventilator than patients without COVID-19. In addition, many of these patients also have a poorly-regulated immune system, where the immune cells damage the organs, but also have impaired anti-microbial functions, increasing the risk of infection.</p> <p>Normally, confirming a pneumonia diagnosis is challenging, as bacterial samples from patients need to be cultured and grown in a lab, which is time-consuming. 探花直播Cambridge test takes an alternative approach by detecting the DNA of different pathogens, which allows for faster and more accurate testing.</p> <p> 探花直播test uses multiple polymerase chain reaction (PCR) which detects the DNA of the bacteria and can be done in around four hours, meaning there is no need to wait for the bacteria to grow. 鈥淥ften, patients have already started to receive antobiotics before the bacteria have had time to grow in the lab,鈥 said Morris. 鈥淭his means that results from cultures are often negative, whereas PCR doesn鈥檛 need viable bacteria to detect 鈥 making this a more accurate test.鈥</p> <p> 探花直播test 鈥 which was developed with Dr Martin Curran, a specialist in PCR diagnostics from Public Health England鈥檚 Cambridge laboratory 鈥 runs multiple PCR reactions in parallel, and can simultaneously pick up 52 different pathogens, which often infect the lungs of patients in intensive care. To be diagnosed with a tumour of any kind is an incredibly traumatic experience for patients. So as clinicians we want to start radiotherapy promptly to improve survival rates and reduce anxiety. It鈥檚 very good at understanding where tumours and healthy organs are. It鈥檚 speeding up the process so I can concentrate on looking at a patient鈥檚 diagnostic images and tailoring treatment to them.</p> <p>鈥淏ut it鈥檚 important for patients to know that the AI is helping me do my job; it鈥檚 not replacing me in the process. I double check everything the AI does and can change it if I need to. 探花直播key thing is that most of the time, I don鈥檛 need to change anything.鈥</p> <p>Up to half of the population in the UK will be diagnosed with cancer at some point in their lives. Of those, half will be treated with radiotherapy, often in combination with other treatments such as surgery, chemotherapy, and increasingly immunotherapy.</p> <p>Radiotherapy involves focusing high-intensity radiation beams to damage the DNA of hard cancerous tumours while avoiding surrounding healthy organs. This is a critical tool, with around 40% of successfully treated patients undergoing some form of radiotherapy.</p> <p>Planning radiotherapy treatment can be a lengthy process. It starts with a 3D CT (Computed Tomography) imaging scan of the part of the body to be targeted. These CT images come in the form of stacks of 2D images, dozens of images deep, each of which must be examined and marked up by a radiation oncologist or specialist technician. This process is called contouring. In each image, an expert must manually draw a contour line around the tumours and key healthy organs at risk in the target area using dedicated computer software. For complex cases, this can take several hours in the planning of a single patient鈥檚 treatment.</p> <p>This image segmentation task is a rate-limiting factor in the cancer treatment pathway for radiotherapy, which increases the burden of time on clinicians and the financial cost to hospitals. As this task is subjective, there can be significant variability across experts and institutions where acquisition protocols and patient demographics vary. This is a limitation to the use of imaging in clinical trials and can introduce variability in patient care.</p> <p>Research published by the team in聽<em>JAMA聽Network Open</em>聽confirms that the聽InnerEye聽ML models can accurately and rapidly carry out the otherwise lengthy 鈥榠mage segmentation鈥 requiring hours of expert clinicians鈥 time.</p> <p>Head of Health Intelligence at Microsoft Research, Aditya Nori, said: 鈥淭his is the first time, we believe, that an NHS Trust has implemented its own deep learning solution trained on their own data, so it can be used on their patients.
