Cambridge Children’s Hospital moves ahead as plans for new hospital approved by ministers

Plans for Cambridge Children’s Hospital can move ahead following the news that the Outline Business Case for the project has been signed off by the Chief Secretary to the Treasury and the Secretary of State for Health and Social Care. ̽��ֱ��project has been given the green light to begin the detailed process of appointing a contractor, to build the ground-breaking new facility in 2026.

̽��ֱ��ministerial backing means that the Project’s Outline Business Case, the second stage of the business case process, has now been fully approved by the Department of Health and Social Care, NHS England and HM Treasury. It was approved in principle in September 2023, subject to a capital affordability review by NHS England and the Department for Health and Social Care’s Joint Investment Committee. That review took place in April 2024 and resulted in a recommendation to Ministers to endorse the decision.

In a show of further confidence in its plans, the Outline Business Case was signed off by the Chief Secretary to the Treasury and the Secretary of State for Health and Social Care in August 2024. This approval recognises that the hospital will meet the needs of patients and staff across the East of England and that the project has the appropriate funding streams in place, to deliver the specialist children’s facility.

̽��ֱ��hospital is being co-designed with the help of children, young people, families and healthcare professionals across the region to ensure the new hospital will meet the needs of patients, families and staff.

Dr Rob Heuschkel, Clinical Lead for Physical Health at Cambridge Children’s Hospital said:

“We are absolutely delighted that we can now move forward to enter contracts with a construction partner, so we can finally start to see work happening on site.

“A huge amount of work has gone into finalising the designs and getting us to this point, and I want to thank our healthcare staff, young people and their families from across the region who have been contributing valuable feedback and helping us shape our plans, right from the very start.

“ ̽��ֱ��East of England is the only region in the UK without a specialist children’s hospital, and we look forward to changing that very soon.”

̽��ֱ��approval comes as a programme of groundworks preparing for the build was completed in July, and new access roads have now been installed where the new five-storey, 35,000sqm hospital will be located, opposite the Rosie Maternity Hospital, on Robinson Way and Dame Mary Archer Way.

In the coming weeks, people will be able to see hoardings installed around the site of Cambridge Children’s Hospital, the first hospital truly designed to bring mental and physical health care together for children and young people.

̽��ֱ��fundraising Campaign for Cambridge Children’s Hospital has now passed the halfway mark and the project remains on track to meet its £100m philanthropy target.

̽��ֱ��hospital will also house a ̽��ֱ�� of Cambridge research institute, focused on preventing childhood illness and early intervention across mental and physical healthcare.

Professor David Rowitch, Clinical Lead for Research at Cambridge Children’s Hospital said:

“By bringing clinicians and patients together with ̽��ֱ�� of Cambridge investigators and industry partners, we aim to shift the medicine paradigm from traditional reactive approaches, to one based on early detection, precision intervention and disease prevention.

“Co-locating research efforts inside Cambridge Children’s Hospital will mean we can detect disease early or even prevent it altogether, personalise health care and prescribe treatments more appropriate for children and their individual health needs.

“We’ll also be able to foster collaborations to advance the power of advanced diagnostics, digital and telehealth technology to support healthcare professions from a distance, to deliver care closer to people’s homes, wherever they live in our region.”

̽��ֱ��Cambridge Children’s Hospital project is a partnership between Cambridge ̽��ֱ�� Hospitals NHS Foundation Trust (CUH), Cambridgeshire and Peterborough NHS Foundation Trust (CPFT), and the ̽��ֱ�� of Cambridge.

AstraZeneca/GSK/ ̽��ֱ�� of Cambridge collaborate to support UK national effort to boost COVID-19 testing

A new testing laboratory will be set up by AstraZeneca, GSK and Cambridge at the ̽��ֱ��’s Anne McLaren Building. This facility will be used for high throughput screening for COVID-19 testing and to explore the use of alternative chemical reagents for test kits in order to help overcome current supply shortages.

Alongside this new testing facility, AstraZeneca and GSK are working together to provide process optimisation support to the UK national testing centres in Milton Keynes, Alderley Park and Glasgow for COVID-19, providing expertise in automation and robotics to help the national testing system to continue to expand capacity over the coming weeks.

While diagnostic testing is not part of either company’s core business, we are moving as fast as we can to help where possible - with a focus on providing our world class scientific and technical expertise - working both with the Government’s screening programme and alongside the wider life sciences sector and specialist diagnostic companies.

Further updates on progress will be issued on this work in due course.

Breathing new life into asthma treatment

I don’t think I will ever forget the moment I sat at the bedside of a six-year-old patient and watched the consultant hand over a 13-year-old student’s design to help with the patient’s asthma treatment. It has been the culmination of a long journey that started eight years ago with the belief that children can solve real world problems as part of the mainstream Design and Technology curriculum.

Designing Our Tomorrow (DOT) is an initiative that puts authentic challenges like this at the heart of the learning experience. Asthma treatment is the epitome of such a challenge. With 5.4 million people in the UK with the condition, the NHS spends about £1 Billion on treatment, and yet 1,468 people died from asthma in 2015. Tragically, it is believed that 90% of these deaths involve preventable factors and similarly 75% of A&E admission are thought to be avoidable.

We set the challenge in schools for students to design a packaging solution that will help co-ordinate the initial treatment for young asthma patients, to put the patient and their carers on the right path to controlling what is typically a long-term condition. A recent survey highlighted that over 80% of people, of all ages, feel that their asthma is not under control. Crucially we wanted students to address the anxiety that a child feels the first time a spacer mask is placed on their face. Students watched a video of a real instance of this, where the child recoils backwards each time the mask is placed over their mouth.

This is a complex, messy problem requiring solutions that are not only effective but cheap, simple to use and scalable. When I first saw the monkey mask design, where the child becomes a monkey and the inhaler and spacer becomes a banana to feed to it, I knew we had something special. It is so simple, so elegant as a design solution, and gets to the very heart of the child’s initial anxiety. Changing that moment from fear to fun for the patient as well as other family members makes it a better experience for all.

Alongside this we have worked with students to develop posters that remind patients to always use their spacer. In addition, we have developed a simple traffic light system explaining the narrowing of airways in the lungs and why and how it can be controlled. ̽��ֱ��credit card-sized printout can be easily clipped to a healthcare professional’s ID badge so it is always to hand.

This is a significant moment in our journey as engineers and educators, and we are so grateful for all the people that have partnered with us on this journey. ̽��ֱ��list of names would be too long, but I do want to mention the organisations that have walked the journey with us. ̽��ֱ��Healthy London Partnership, Children and Young People’s programme (a collaboration of the health and social care system across London), whose passion and skill around asthma has been an inspiration. On the packaging side the British Printing Industry Federation (BPiF) and ̽��ֱ��Institute of Materials, Minerals and Mining (IOM3) who have guided us in the realities of packaging design and production; DS Smith who turned around amazing designs in such short timescales; Peter Brett Associates who believed in the project when it was just an idea; and last, but by no means least, the teachers and students who brought it to life in the classroom.

This was perfectly timed to fit with the Healthy London Partnership #AskAbout Asthma campaign and our pledge is to run the ‘Unpacking Asthma’ challenge in schools again in this academic year. We are confident that students will come up with more ideas that can help with this vital work.

It’s hard not to sound corny, but Churchill’s words come to mind for the vision we had for DOT eight years ago, “now this is not the end. It is not even the beginning of the end. But it is, perhaps, the end of the beginning”. We hope that this work will go on to play a part in transforming asthma care and help us towards our goal of equipping future generations to be creative problem solvers. In other words, for young people to design a better tomorrow.

No evidence to support claims that telephone consultations reduce GP workload or hospital referrals

As UK general practices struggle with rising demand from patients, more work being transferred from secondary to primary care, and increasing difficulty in recruiting general practitioners, one proposed potential solution is a ‘telephone first’ approach, in which every patient asking to see a GP is initially phoned back by their doctor on the same day. At the end of this phone call the GP and the patient decide whether the problem needs a face-to-face consultation, or whether it has been satisfactorily resolved on the phone.

Two commercial companies provide similar types of management support for practices adopting the new approach, with claims that the approach dramatically reduces the need for face-to-face consultations, reduces workload stress for GPs and practice staff, increases continuity of care, reduces A&E attendance and emergency hospital admissions, and increases patient satisfaction.

Some of these claims are repeated in NHS England literature, including the assertion based on claims from one of the companies that practices using the approach have a 20% lower A&E usage and that “the model has demonstrated a cost saving of approximately £100k per practice through prevention of avoidable attendance and admissions to hospital”. Several Clinical Commissioning Groups have subsequently paid for the management support required for the approach to be adopted by practices in their area.

̽��ֱ��National Institute for Health Research (NIHR) acknowledged the need for robust and independent evaluation of current services and therefore commissioned the team led by Martin Roland, Emeritus Professor of Health Services Research at the ̽��ֱ�� of Cambridge. ̽��ֱ��results of the evaluation, which looked at data sources including GP and hospital records, patient surveys and economic analyses, are published today in ̽��ֱ��BMJ.

̽��ֱ��study found that adoption of the ‘telephone first’ approach had a major effect on patterns of consultation: the number of telephone consultations increased 12-fold, and the number of face-to-face consultations fell by 38%.

However, the study found that the ‘telephone first’ approach was on average associated with increased overall GP workload; there was an overall increase of 8% in the mean time spent consulting by GPs, but this figure masks a wide variation between practices, with some practices experiencing a substantial reduction in workload and others a large increase.

Dr Jennifer Newbould from RAND Europe, part of the Cambridge Centre for Health Services Research, the study’s first author, says: “There are some positives to a ‘telephone first approach’; for example, we found clear evidence that a significant part of patient workload can be addressed through phone consultations. But we need to be careful about seeing this as a panacea: while this may increase a GP practice’s control over day-to-day workload, it does not necessarily decrease the amount of time GPs spend consulting and may, in some cases, increase it.”

̽��ֱ��researchers found no evidence that the approach substantially reduced overall attendance at A&E departments or emergency hospital admissions: introduction of the ‘telephone first’ approach was followed by a small (2%) increase in hospital admissions, no initial change in A&E attendance, but a small (2% per year) decrease in the subsequent rate of rise of A&E attendance. However, far from reducing secondary care costs, they found overall secondary care costs increased slightly by £11,776 per 10,000 patients.

Professor Roland adds: “Importantly, we found no evidence to support claims made by one of the companies that support such services – claims that have been repeated by NHS England – that the approach would be substantially cost-saving or reduce hospital referrals. This has resulted in some Clinical Commissioning Groups across England buying their consultancy services based on unsubstantiated claims. ̽��ֱ��NHS must be careful to ensure that it bases its information and recommendation on robust evidence.”

Student-led designs could help prevent childhood asthma deaths

̽��ֱ��programme, called Designing Our Tomorrow, was founded by researchers at the ̽��ֱ�� of Cambridge, and brings real-world problems into classroom design and technology sessions in secondary schools, and encouraging the next generation of UK designers and engineers.

As part of their classroom curriculum, students from different secondary schools have been learning about what makes an effective and useful design. Their goal was to design a type of packaging which would contain everything a young child with asthma would need, whether they’re at home, at school or elsewhere; and one which would help reduce anxiety of children with asthma by using child-friendly design themes. “In other words, we want to make it fun,” said Ian Hosking of Cambridge’s Department of Engineering, who co-leads the Designing Our Tomorrow (DOT) programme, in collaboration with the Faculty of Education. “We want to re-frame what education can be – projects like these start to form a broader evidence base of what’s possible.”

Five of the best designs were presented by students from Wimbledon High School GDST at the British Paediatric Respiratory Society conference last Friday (30 June) in Cambridge.

Students were not merely designing packaging but an experience. Themes included a monkey character where the inhaler and spacer become a banana that the child can ‘feed’ the monkey with and then copy themselves. Other themes include a pack shaped like a cat where the inhalers become mice that are stored in a smaller box shaped like a wedge of cheese; and a folding pack that can hang on a door for easy access at home but can be quickly zipped up and put in a bag to take out.

“Seeing how people were scared of asthma…this affects and could benefit a lot of people. ̽��ֱ��child wanted it to be fun, the gran wanted emergency instructions, the parents wanted it to be compact and small and the nurse wanted it to be organised – so we took all of that and designed our packs,” said Charlotte, aged 11 from Wimbledon High.

Several of the designs have been made into initial corrugated cardboard prototypes by UK packaging company DS Smith, with the aim of piloting them in partnership with the NHS in London through the Healthy London Partnership.

“It has been great doing something which is able to change and improve children’s lives and help them get better,” said Sascha, aged 12 from Wimbledon High, one of the students who presented her design at the conference. “I am so happy and glad that they have decided to take mine to the next stage and it could appear in people’s homes.”

Asthma affects one in 11 children in the UK. On average, there are three children with asthma in every classroom in the UK, and a child is admitted to hospital every 20 minutes due to an asthma attack.

This DOT project has focused specifically on asthma in children under six years of age. It addresses the anxiety that a child feels in the early stages of treatment and the co-ordination of the equipment and their instructions to help ensure compliance with their treatment plan.

“DOT is a fascinating project which aims to bring real-world problems into classroom design and technology sessions in secondary schools,” said Sara Nelson from the Healthy London Partnership. “It’s one of the more rewarding pieces of work that I have had the pleasure of being involved in during the last year, the one I have learned the most from, and it involved collaborating with an unusual partner for the NHS.”

Each of the students was given all of the tools which a child with asthma or their carer would need to manage their condition, including inhalers, spacers, and emergency instructions. Through a set of classroom lessons, the students’ way of thinking was developed in order to help them understand how to be creative by breaking fixation through the use of stimulus.

Fixation is a common problem in design – for example, if you’re trying to design a new type of chair and all you’re shown are other chairs, you’ll just end up designing a variant of what already exists. “If I want to design a new chair, the last thing I should look at is a chair,” said Bill Nicholl from Cambridge’s Faculty of Education, who co-leads the DOT programme.

“Children and young people are terribly creative, and the NHS should involve them more in co-designing what we do,” said Nelson. “We should not be afraid to put our heads above the parapet and should look outside the NHS to what partnerships might be out there to help us solve some of our tricky problems – I know that we can learn an awful lot from engineers.”

̽��ֱ��students from Wimbledon High also gained valuable industry experience working with DS Smith, who will help refine the students’ concepts into something that can be manufactured in large volumes. “By working with industry, it takes the project beyond a competition to something that can make a difference to patients and help prevent avoidable asthma deaths in children,” said Hosking.

“This project has shown yet again the potential of young people and their ability to engage with, and ultimately solve, complex design problems. We underestimate their creativity at our peril. Solving real problems like this should be at the heart of all young people’s educational experiences,” said Nicholl.

Wimbledon High Head of Design & Technology Marcia Phillip has been deeply involved in the project: “It puts authentic challenges and engineering practice at the heart of the learning experience and this appealed to me, particularly working in a girls’ school. We know there is a shortage of engineers in the UK – particularly women – and I thought we should get our girls inspired from an early age. ̽��ֱ��girls have been highly engaged and excited – after all, they are playing a part in the ̽��ֱ�� of Cambridge’s research and their ideas will potentially be implemented within the NHS.”

“I feel like I am doing something for a purpose and it makes me feel happy that I am helping people,” said Charlotte. “I feel accomplished and proud of what I have done because it was a long process but it was all worth it.”

DOT is funded in part by engineering design consultants Peter Brett Associates and ARM Ltd.

Solutions designed by secondary school students as part of an innovative classroom design and technology programme could help reduce the number of unnecessary deaths from childhood asthma.

Cambridge has received funding for two units under the £12.1 million scheme. ̽��ֱ��Units will be centres of excellence in human experimental medicine related to blood and transplantation and will have a strong focus on translation. They will support the delivery of objectives and functions of NHS Blood and Transplant, by creating an environment where world-class research, focused on the organisation’s needs, can thrive, and will provide high quality research evidence to inform decision making at NHS Blood and Transplant.

Speaking about the partnership funding awards, Dr Lorna Williamson, Medical and Research Director at NHS Blood and Transplant, said: "I am delighted that the Department of Health, through the NIHR, continues to recognise the importance of blood and transplantation research. This funding supports ambitious experimental research projects that will inform future clinical practice for services that NHS Blood and Transplant provides to the NHS and beyond."

Professor Andrew Bradley, Head of the Department of Surgery at the ̽��ֱ�� of Cambridge, in partnership with Professor Andrew Fisher from Newcastle ̽��ֱ��, will establish a unit focused on organ donation and transplantation. ̽��ֱ��Cambridge/Newcastle unit will focus on understanding how to improve the quality of organs prior to donation and will develop and evaluate novel approaches and technologies that increase the availability of suitable donor organs for transplantation, while improving graft survival.

Professor John Danesh from the Cambridge Institute of Public Health will lead a unit focused on donor health and genomics, a new area of research for NHS Blood and Transplant. ̽��ֱ��Unit will address major questions about the health of blood donors and produce evidence-based strategies to enhance donor safety while ensuring sustainability of blood supply.

̽��ֱ��Units will be based at Addenbrooke’s Hospital, part of the Cambridge ̽��ֱ�� Hospitals Partnership, and located within the Cambridge Biomedical Campus, the centrepiece of the largest biotech cluster outside the United States.

Professor Bradley said: “Blood and transplantation research is vital to improving the quality, safety and availability of donation and transplantation. These two new NIHR units will play an important role in this area and inform NHS policy and practice in the future. They will further add to and capitalise on continuing growth of the Cambridge Biomedical Campus.”

Professor Dame Sally C Davies FRS FMedSci, Chief Medical Officer and Chief Scientific Adviser at the Department of Health, said: “ ̽��ֱ��NHS and its patients rely on an efficient supply of blood and organ donations and, increasingly, stem cells and genomics. We want researchers to explore how to improve the quality and effectiveness of these donations, therapies and technologies. ̽��ֱ��NIHR Blood and Transplant Research Units will involve NHSBT in partnerships with leading university teams so that we can accelerate and translate advances in research into benefits for donors and patients.”

A third unit is due to open at UCL ( ̽��ֱ�� College London), led by Dr Karl Peggs and focused on Stem Cells and Immunotherapies.

̽��ֱ�� ̽��ֱ�� of Cambridge has received £7.9 million from the National Institute for Health Research (NIHR) to fund Blood and Transplant Research Units. Each Unit is a partnership between ̽��ֱ�� researchers and NHS Blood and Transplant, and will begin in October 2015.
