It is an aggressive form of blood cancer that is very difficult to treat. Thanks to recent advances, individuals at high risk of AML can be identified years in advance using blood tests and blood DNA analysis, but there鈥檚 no suitable treatment that can prevent them from developing the disease.</p> <p>In this study, Professor George Vassiliou and colleagues at the 探花直播 of Cambridge investigated how to prevent abnormal blood stem cells with genetic changes from progressing to become AML. 探花直播work focused on the most common genetic change, which affects a gene called DNMT3A and is responsible for starting 10-15% of AML cases.</p> <p>Professor Vassiliou, from the Cambridge Stem Cell Institute at the 探花直播 of Cambridge and聽Honorary Consultant Haematologist at Cambridge 探花直播 Hospitals NHS Foundation Trust (CUH) co-led the study. He said: 鈥淏lood cancer poses unique challenges compared to solid cancers like breast or prostate, which can be surgically removed if identified early. With blood cancers, we need to identify people at risk and then use medical treatments to stop cancer progression throughout the body.鈥</p> <p> 探花直播research team examined blood stem cells from mice with the same changes in DNMT3A as seen in the pre-cancerous cells in humans. Using a genome-wide screening technique, they showed that these cells depend more on mitochondrial metabolism than healthy cells, making this a potential weak spot. 探花直播researchers went on to confirm that metformin, and other mitochondria-targeting drugs, substantially slowed the growth of mutation-bearing blood cells in mice. Further experiments also showed that metformin could have the same effect on human blood cells with the DNMT3A mutation.</p> <p>Dr Malgorzata Gozdecka, Senior Research Associate at the Cambridge Stem Cell Institute and first author of the research said: 鈥淢etformin is a drug that impacts mitochondrial metabolism, and these pre-cancerous cells need this energy to keep growing. By blocking this process, we stop the cells from expanding and progressing towards AML, whilst also reversing other effects of the mutated DNMT3A gene.鈥</p> <p>In addition, the study looked at data from over 412,000 UK Biobank volunteers and found that people taking metformin were less likely to have changes in the DNMT3A gene. This link remained even after accounting for factors that could have confounded the results such as diabetes status and BMI.</p> <p>Professor Brian Huntly, Head of the Department of Haematology at the 探花直播 of Cambridge,聽Honorary Consultant Haematologist at CUH, and joint lead author of the research, added: 鈥淢etformin appears highly specific to this mutation rather than being a generic treatment. That specificity makes it especially compelling as a targeted prevention strategy.</p> <p>鈥淲e鈥檝e done the extensive research all the way from cell-based studies to human data, so we鈥檙e now at the point where we have a made a strong case for moving ahead with clinical trials. Importantly, metformin鈥檚 lack of toxicity will be a major advantage as it is already used by millions of people worldwide with a well-established safety profile.鈥</p> <p> 探花直播results of the study, funded by Blood Cancer UK with additional support from Cancer Research UK, the Leukemia &amp; Lymphoma Society (USA) and the Wellcome Trust, are published in Nature.</p> <p>Dr Rubina Ahmed, Director of Research at Blood Cancer UK, said: 鈥淏lood cancer is the third biggest cancer killer in the UK, with over 280,000 people currently living with the disease. Our Blood Cancer Action plan shed light on the shockingly low survival for acute myeloid leukaemia, with only around 2 in 10 surviving for 5 years, and we urgently need better strategies to save lives. Repurposing safe, widely available drugs like metformin means we could potentially get new treatments to people faster, without the need for lengthy drug development pipelines.鈥</p> <p> 探花直播next phase of this research will focus on clinical trials to test metformin鈥檚 effectiveness in people with changes in DNMT3A at increased risk of developing AML. 聽With metformin already approved and widely used for diabetes, this repurposing strategy could dramatically reduce the time it takes to bring a new preventive therapy to patients.<br /> <br /> Tanya Hollands, Research Information Manager at Cancer Research UK, who contributed funding for the lab-based screening in mice, said: 鈥淚t's important that we work to find new ways to slow down or prevent AML in people at high risk. They used that data from Genomics England鈥檚 100,000 Genomes Project to create an algorithm capable of identifying tumours with faults in their DNA that makes them easier to treat.</p> <p> 探花直播algorithm, called PRRDetect, could one day help doctors work out which patients are more likely to have successful treatment. That could pave the way for more personalised treatment plans that increase people鈥檚 chances of survival.</p> <p> 探花直播research was funded by Cancer Research UK and the National Institute for Health and Care Research (NIHR).</p> <p>Professor Serena Nik-Zainal 聽from the Early Cancer Institute at the 探花直播 of Cambridge, lead author of the study, said: 鈥淕enomic sequencing is now far faster and cheaper than ever before. We are getting closer to the point where getting your tumour sequenced will be as routine as a scan or blood test.</p> <p>鈥淭o use genomics most effectively in the clinic, we need tools which give us meaningful information about how a person鈥檚 tumour might respond to treatment. This is especially important in cancers where survival is poorer, like lung cancer and brain tumours.</p> <p>鈥淐ancers with faulty DNA repair are more likely to be treated successfully. PRRDetect helps us better identify those cancers and, as we sequence more and more cancers routinely in the clinic, it could ultimately help doctors better tailor treatments to individual patients.鈥</p> <p> 探花直播research team looked for patterns in DNA created by so-called 鈥榠ndel鈥 mutations, in which letters are inserted or deleted from the normal DNA sequence. 聽</p> <p>They found unusual patterns of indel mutations in cancers that had faulty DNA repair mechanisms 鈥 known as 鈥榩ost-replicative repair dysfunction鈥 or PRRd. Using this information, the scientists developed PRRDetect to allow them to identify tumours with this fault from a full DNA sequence.</p> <p>PRRd tumours are more sensitive to immunotherapy, a type of cancer treatment that uses the body鈥檚 own immune system to attack cancer cells. 探花直播scientists hope that the PRRd algorithm could act like a 鈥榤etal detector鈥 to allow them to identify patients who are more likely to have successful treatment with immunotherapy.</p> <p> 探花直播study follows from a previous 鈥榓rchaeological dig鈥 of cancer genomes carried out by Professor Nik-Zainal, which examined the genomes of tens of thousands of people and revealed previously unseen patterns of mutations which are linked to cancer.</p> <p>This time, Professor Nik-Zainal and her team looked at cancers which have a higher proportion of tumours with PRRd. These include bowel, brain, endometrial, skin, lung, bladder and stomach cancers. Whole genome sequences of these cancers were provided by the 100,000 Genomes Project - a pioneering study led by Genomics England and NHS England which sequenced 100,000 genomes from around 85,000 NHS patients affected by rare diseases or cancer.</p> <p> 探花直播study identified 37 different patterns of indel mutations across the seven cancer types included in this study. Ten of these patterns were already linked to known causes of cancer, such as smoking and exposure to UV light. Eight of these patterns were linked to PRRd. 探花直播remaining 19 patterns were new and could be linked to causes of cancer that are not fully understood yet or mechanisms within cells that can go wrong when a cell becomes cancerous.</p> <p>Executive Director of Research and Innovation at Cancer Research UK, Dr Iain Foulkes, said: 鈥淕enomic medicine will revolutionise how we approach cancer treatment. We can now get full readouts of tumour DNA much more easily, and with that comes a wealth of information about how an individual鈥檚 cancer can start, grow and spread.</p> <p>鈥淭ools like PRRDetect are going to make personalised treatment for cancer a reality for many more patients in the future. Alongside this, the programme will develop powerful new tools that use AI to analyse the data and calculate an individual鈥檚 risk of cancer throughout their lifetime.</p> <p>Professor Antoniou said: 鈥淔inding people at the highest risk of developing cancer, including those with vague symptoms, is a major challenge. 探花直播UK鈥檚 strengths in population-scale data resources, combined with advanced analytical tools like AI, offer tremendous opportunities to link disparate datasets and uncover clues that could lead to earlier detection, diagnosis, and prevention of more cancers.鈥</p> <p>Over the next five years, the funding will build the infrastructure required to access and link these datasets, train new data scientists, create the algorithms behind the risk models and evaluate the algorithms and AI tools to ensure that they are giving accurate and clinically useful information about cancer risk. 探花直播scientific programme will be guided by partnerships with cancer patients, the public, clinical experts and industry, while addressing ethical and legal considerations to ensure that the models and tools work well in practice.</p> <p>Professor Antoniou added: 鈥淯ltimately, [the Cancer Data Driven Detection programme] could inform public health policy and empower individuals and their healthcare providers to make shared decisions. By understanding individual cancer risks, people can take proactive steps to stop cancer before it gets worse or even begins in the first place.鈥</p> <p> 探花直播models generated from this research could be used to help people at higher risk of cancer in different ways. For example, the NHS could offer more frequent cancer screening sessions or screening at a younger age to those at higher risk, whilst those at lower risk could be spared unnecessary tests. People identified as higher risk could also be sent for cancer testing faster when they go to their GP with possible cancer signs or symptoms. Individuals at higher risk could also access different ways to prevent cancer.</p> <p>Earlier diagnosis of cancer saves lives, yet according to analysis of NHS figures by Cancer Research UK, only 54% of cancers in England are diagnosed at stages one and two, where treatment is more likely to be successful. NHS England has set a target to diagnose 75% of cancers at stages one and two by 2028, and this will only be achieved with research and embracing new technologies to catch cancer earlier. 聽</p> <p>Last week, the Prime Minister announced backing for the power of big data and AI, which has the potential to help even more patients, including those with cancer.</p> <p>Science Minister Lord Vallance said: 鈥淭here are huge opportunities in AI to improve UK healthcare, from scans detecting illnesses earlier to bringing NHS waiting lists down by planning appointments more efficiently, and these will continue to develop.</p> <p>鈥淭his investment in harnessing the potential of data to spot those at risk of cancer represents the sort of innovation the Government鈥檚 new AI Opportunities Action Plan sets out to realise, so this technology improves lives, while transforming public services and boosting growth.鈥</p> <p>Minister for Public Health and Prevention, Andrew Gwynne said:聽鈥淯sing the latest technology could revolutionise how the NHS diagnoses and treats patients. Around 36,600 lives are saved from lung cancer in the UK every year, according to new analysis from Cancer Research UK.</p> <p>Professor Ljiljana Fruk and Dr Daniel Munoz Espin and their teams at the 探花直播 of Cambridge are leading on the research, funded by Cancer Research UK.</p> <p> 探花直播work, at Cambridge鈥檚 Department of Chemical Engineering and Biotechnology, and the Early Cancer Institute, will provide a cheap, affordable sensor that uses urine samples to help doctors detect lung cancer before the disease develops.</p> <p>Lung cancer has a poor prognosis for many patients because often there are no noticeable symptoms until it has spread through the lungs or into other parts of the body. 探花直播new urine test will allow doctors to spot the disease before it develops.</p> <p>To create the test, scientists looked at proteins excreted by senescent cells: 鈥渮ombie鈥 cells which are alive but unable to grow and divide. It鈥檚 these cells that cause tissue damage by reprogramming their immediate environment to help promote the emergence of cancer cells.</p> <p>Now, researchers have developed an injectable sensor that interacts with zombie cell proteins and releases easily detectable compound into urine, signalling their presence.</p> <p>鈥淓arly detection of cancer requires cost-effective tools and strategies that enable detection to happen quickly and accurately,鈥 said Fruk. 鈥淲e designed a test based on peptide-cleaving proteins, which are found at higher levels in the presence of zombie cells, and in turn appear in the early stages of cancer.</p> <p>鈥淯ltimately, we want to develop a urine test that could help doctors identify signs of the early stages of cancer 鈥 potentially months or even years before noticeable symptoms appear.鈥</p> <p>As well as targeting lung cancer, Fruk聽hopes her research, along with joint efforts across other university departments, will result in the development of probes capable of detecting other cancers.</p> <p>鈥淲e have almost completed a functional urine test to detect 鈥榸ombie' cells in lung cancer, which will spot cancer earlier and avoid the need for invasive procedures, but this test does have potential for other cancers,鈥 she said. 鈥淒eveloping more efficient cancer treatments requires earlier detection and better therapies, but also work with other disciplines for a more holistic view of the disease, which is an essential part of my research.鈥</p> <p>From uncovering the causes of lung cancer to pioneering drugs to treat it, Cancer Research UK has helped power progress for people affected by lung cancer. This is helping scientists to predict how well immunotherapy treatments will work.</li> <li>Cancer detection: Scientists are finding new ways to detect cancer earlier, predict the best course of treatments and tailor therapies to individual needs, to improve survival.鈥 Using tumour DNA, scientists can monitor鈥痶he effectiveness of treatments and catch signs of cancer returning. 聽Cambridge scientists are also working on a simple at-home test for future patients to regularly monitor their progress.聽</li> <li>Personalised medicine: Looking at the unique genetic mutations of a person鈥檚 tumour, including how it behaves and responds to treatment, allows treatments to be developed and matched to the specific genetic change.鈥 For example, Professor James Brenton's team discovered a specific mutation in鈥痶he most common form of ovarian cancer which is now used across the NHS as a cancer marker to measure treatment response for the disease.聽</li> </ul> <p>Thanks to research, cancer death rates have fallen by 10% percent in the UK over the past decade. Now, other scientists will be able to access these technologies to create detailed tumour maps that could one day transform how we diagnose and treat cancer.</p> <p>Led by Professor Greg Hannon and Dr Dario Bressan at the Cancer Research UK Cambridge Institute and Dr Nicholas Walton at the 探花直播 of Cambridge鈥檚 Institute of Astronomy, SPACE will give other researchers the opportunity to study cancer in a way that wasn鈥檛 previously possible.</p> <p>Dr Dario Bressan, Head of the SPACE Laboratory at the Cancer Research UK Cambridge Institute, said:聽鈥淭umours aren鈥檛 just a uniform mass of cells; they consist of a diverse ecosystem of cancer cells, immune cells, and other essential components that support their survival. Hidden within these intricate networks lies valuable information which could guide us in making more personalised treatment decisions for each patient.聽</p> <p>鈥淲ith the SPACE platform, researchers can zoom into specific cell populations, highlight the complex connections between them, and even run virtual experiments to predict how the tumour might respond to different treatments. By unlocking these insights, we can transform the future of cancer care and uncover new opportunities for targeted therapies.鈥澛</p> <p> 探花直播IMAXT team was first awarded 拢20 million in 2017 by Cancer Research UK through Cancer Grand Challenges, a global research initiative co-founded by Cancer Research UK and the National Cancer Institute in the US.</p> <p>Since then, the team has united experts from fields rarely brought together including medicine, Virtual Reality (VR), programming, molecular biology, chemistry, mathematics, and even astronomy, to create a completely immersive tool for studying tumours.</p> <p>As well as enabling scientists to analyse 3D tumour maps, IMAXT created pioneering VR technology which allows the user to 鈥榮tep inside鈥 a tumour using a VR headset.</p> <p>With the headset, scientists get to view vast amounts of detailed data about individual tumour cells in a 3D space. Instead of looking at this data on a computer screen, they can see all the information in real-time, as if they were inside the tumour itself.</p> <p>Professor Greg Hannon, Director of the Cancer Research UK Cambridge Institute, said: "Cancer Grand Challenges offers a unique opportunity for international teams to address some of cancer鈥檚 biggest challenges. Additional support for the SPACE project has been provided by the UK Space Agency through their funding of the development of imaging and analysis techniques at the IoA, Cambridge for a range of space science missions.
