Unlike a stethoscope, the device works well even if it鈥檚 not placed precisely on the chest: its larger, flexible sensing area helps capture clearer heart sounds than traditional stethoscopes.</p> <p> 探花直播device can also be used over clothing, making it more comfortable for patients 鈥 especially women 鈥 during routine check-ups or community heart health screening programmes.</p> <p> 探花直播heart sound recordings can be saved on the device, which can then be used to detect signs of heart valve disease. 探花直播researchers are also developing a machine learning algorithm which can detect signs of valve disease automatically. 探花直播<a href="https://ieeexplore.ieee.org/document/10930505">results</a> are reported in the <em>IEEE Journal of Biomedical and Health Informatics</em>.</p> <p>Heart valve disease (valvular heart disease or VHD) has been called the 鈥榥ext cardiac epidemic,鈥 with a prognosis worse than many forms of cancer. Up to 50% of patients with significant VHD remain undiagnosed, and many patients only see their doctor when the disease has advanced and they are experiencing significant complications.</p> <p>In the UK, the NHS and NICE have identified early detection of heart valve disease as a key goal, both to improve quality of life for patients, and to decrease costs.</p> <p>An examination with a stethoscope, or auscultation, is the way that most diagnoses of heart valve disease are made. However, just 38% of patients who present to their GP with symptoms of valve disease receive an examination with a stethoscope.</p> <p>鈥 探花直播symptoms of VHD can be easily confused with certain respiratory conditions, which is why so many patients don鈥檛 receive a stethoscope examination,鈥 said Professor Anurag Agarwal from Cambridge鈥檚 Department of Engineering, who led the research. 鈥淗owever, the accuracy of stethoscope examination for diagnosing heart valve disease is fairly poor, and it requires a GP to conduct the examination.鈥</p> <p>In addition, a stethoscope examination requires patients to partially undress, which is both time consuming in short GP appointments, and can be uncomfortable for patients, particularly for female patients in routine screening programmes.</p> <p> 探花直播鈥榞old standard鈥 for diagnosing heart valve disease is an echocardiogram, but this can only be done in a hospital and NHS waiting lists are extremely long 鈥 between six to nine months at many hospitals.</p> <p>鈥淭o help get waiting lists down, and to make sure we鈥檙e diagnosing heart valve disease early enough that simple interventions can improve quality of life, we wanted to develop an alternative to a stethoscope that is easy to use as a screening tool,鈥 said Agarwal.</p> <p>Agarwal and his colleagues have developed a handheld device, about the diameter of a drinks coaster, that could be a solution. Their device can be used by any health professional to accurately record heart sounds, and can be used over clothes.</p> <p>While a regular or electronic stethoscope has a single sensor, the Cambridge-developed device has six, meaning it is easier for the doctor or nurse 鈥 or even someone without any medical training 鈥 to get an accurate reading, simply because the surface area is so much bigger.</p> <p> 探花直播device contains materials that can transmit vibration so that it can be used over clothes, which is particularly important when conducting community screening programmes to protect patient privacy. Between each of the six sensors is a gel that absorbs vibration, so the sensors don鈥檛 interfere with each other.</p> <p> 探花直播researchers tested the device on healthy participants with different body shapes and sizes and recorded their heart sounds. Their next steps will be to test the device in a clinical setting on a variety of patients, against results from an echocardiogram.</p> <p>In parallel with the development of the device, the researchers have developed a machine learning algorithm that can use the recorded heart sounds to detect signs of valve disease automatically. Early tests of the algorithm suggest that it outperforms GPs in detecting heart valve disease. 聽</p> <p>鈥淚f successful, this device could become an affordable and scalable solution for heart health screening, especially in areas with limited medical resources,鈥 said Agarwal.</p> <p> 探花直播researchers say that the device could be a useful tool to triage patients who are waiting for an echocardiogram, so that those with signs of valve disease can be seen in a hospital sooner.</p> <p>A patent has been filed on the device by Cambridge Enterprise, the 探花直播鈥檚 commercialisation arm. Some of these materials will come from outside, such as bacterial or viral infections, while others can be produced within the body.</p>&#13; &#13; <p>Innate immunity triggers an inflammatory response, which aims to attack and destroy the perceived threat. But sometimes, this response can become overactive and can itself cause harm to the body.</p>&#13; &#13; <p>One such example of this is gout, which occurs when urate crystals build up in joints, causing excessive inflammation, leading to intense pain. Another example is heart attack, where dead cell build up in the damaged heart 鈥 the body sees itself as being under attack and an overly-aggressive immune system fights back, causing collateral damage to the heart.</p>&#13; &#13; <p>Several of these conditions are characterised by overactivation of a component of the innate immune response known as an inflammasome 鈥 specifically, the inflammasome NLRP3. Scientists at the Victor Phillip Dahdaleh Heart and Lung Research Institute at Cambridge have found a molecule that helps NLRP3 respond.</p>&#13; &#13; <p>This molecule is known as PLK1. It is involved in a number of processes within the body, including helping organise tiny components of our cells known as microtubules cytoskeletons. These behave like train tracks inside of the cell, allowing important materials to be transported from one part of the cell to another.</p>&#13; &#13; <p>Dr Xuan Li from the Department of Medicine at the 探花直播 of Cambridge, the study鈥檚 senior author, said: 鈥淚f we can get in the way of the microtubules as they try to organise themselves, then we can in effect slow down the inflammatory response, preventing it from causing collateral damage to the body. We believe this could be important in preventing a number of common diseases that can cause pain and disability and in some cases can lead to life-threatening complications.鈥</p>&#13; &#13; <p>But PLK1 also plays another important role in the body 鈥 and this may hold the key to developing new treatments for inflammatory diseases.</p>&#13; &#13; <p>For some time now, scientists have known that PLK1 is involved in cell division, or mitosis, a process which, when it goes awry, can lead to runaway cell division and the development of tumours. This has led pharmaceutical companies to test drugs that inhibit its activity as potential treatments for cancer. At least one of these drugs is in phase three clinical trials 鈥 the final stages of testing how effective a drug is before it can be granted approval.</p>&#13; &#13; <p>When the Cambridge scientists treated mice that had developed inflammatory diseases with a PLK1 inhibitor, they showed that it prevented the runaway inflammatory response 鈥 and at a much lower dose than would be required for cancer treatment. In other words, inhibiting the molecule 鈥榗almed down鈥 NLRP3 in non-dividing cells, preventing the overly aggressive inflammatory response seen in these conditions.</p>&#13; &#13; <p> 探花直播researchers are currently planning to test its use against inflammatory diseases in clinical trials.</p>&#13; &#13; <p>鈥淭hese drugs have already been through safety trials for cancer 鈥 and at higher doses than we think we would need 鈥 so we鈥檙e optimistic that we can minimise delays in meeting clinical and regulatory milestones,鈥 added Dr Li.</p>&#13; &#13; <p>鈥淚f we find that the drug is effective for these conditions, we could potentially see new treatments for gout and inflammatory heart diseases 鈥 as well as a number of other inflammatory conditions 鈥 in the not-too-distant future.鈥</p>&#13; &#13; <p> 探花直播research was funded by the British Heart Foundation.聽Professor James Leiper, Associate Medical Director at the British Heart Foundation said:聽鈥淭his innovative research has uncovered a potential new treatment approach for inflammatory heart diseases such as heart failure and cardiomyopathy. Cardiovascular and lung diseases kill more than 26 million people a year and have a major impact on the quality of life of many more. Alongside the immense human cost, the economic burden of these diseases 鈥 an estimated annual global cost of 拢840 billion 鈥 is already overwhelming and unsustainable. However, many drugs that make failing heart muscle contract more strongly have been deemed unsafe, leaving a huge gap in heart attack and heart failure treatment. Scientists now believe that they might have identified a new drug target 鈥 a protein called MARK4.</p> <p>In research funded by the British Heart Foundation (BHF), Cambridge scientists found levels of MARK4 were elevated in mouse hearts after a heart attack. When they compared mice with and without MARK4 in the heart, they found hearts without the protein were 57 per cent better at pumping blood. This protective effect was seen 24 hours after a heart attack and lasted for the entire follow-up period of four weeks.</p> <p> 探花直播team identified for the first time that MARK4 fine-tunes a structural network within the heart muscle cell 鈥 called the microtubule network 鈥 that attaches to the machinery which makes heart muscle cells contract and relax. When MARK4 levels were increased after a heart attack, microtubules were tightly anchored onto the contractile machinery in the heart, causing more resistance and preventing them from functioning normally. When MARK4 levels were reduced, microtubules were loosely anchored, making it easier for the heart to contract and relax.</p> <p>After a heart attack, in the heart muscle cells of mice without MARK4, the speed of contraction increased by 42 per cent and the speed of relaxation increased by 47 per cent, compared to muscle cells from mice with the MARK4 protein. They were also close to functioning at the same level as healthy heart muscle cells, showing the power of lowering levels of MARK4.</p> <p>Now, the researchers suggest that drugs to switch off MARK4 could provide a promising new way to improve recovery and help the heart to pump blood more efficiently in people with failing hearts.</p> <p>Dr Xuan Li, BHF Intermediate Research Fellow at 探花直播 of Cambridge BHF Centre of Research Excellence, said: 鈥淎fter years of research we鈥檝e revealed an entirely new and promising way that could help the recovery of failing hearts.</p> <p>鈥淚t鈥檚 early days, and we now need to test the longer-term effects of switching off MARK4. But if drugs to do that prove successful, the life-changing benefits could be seen in people with other types of heart disease as well as those who鈥檝e had a heart attack and developed heart failure.鈥</p> <p>Professor Metin Avkiran, Associate Medical Director at the British Heart Foundation, said: 鈥淗eart attacks are a major cause of disability worldwide - people who鈥檝e had a major heart attack are at much greater risk of developing chronic heart failure. There are around 920,000 people living with heart failure in the UK, and we desperately need drugs to drastically improve the heart鈥檚 function in these patients.</p> <p>鈥淭hese findings are a positive step forward. Autistic adults were also far more likely to have atypical eating patterns (including limited diet) and sleep disturbance. They were more likely to be underweight or obese than non-autistic individuals.</p>&#13; &#13; <p>These poor lifestyle habits were associated with greater risk of cardiovascular conditions such as high blood pressure, heart disease, and stroke among autistic males, and this was a stronger association even than a family history of a cardiovascular condition. Though it is not possible to say conclusively that a poorer lifestyle led to cardiovascular problems, the findings provide the first indication that promoting healthy choices regarding diet, exercise, and sleep may help to reduce the excess risks of health conditions in autistic adults.聽</p>&#13; &#13; <p>While the results indicate that there may be other biological or environmental factors that leave autistic individuals at greater risk of health conditions, they also provide a clear target for intervention. Difficulties with maintaining a healthy lifestyle may also have knock-on effects beyond physical health, including limiting opportunities for social interaction (which may centre around mealtimes or exercise), and could contribute to worsening mental health, and affect employment or education.</p>&#13; &#13; <p> 探花直播lead researcher of the study, Elizabeth Weir, a PhD student at the Autism Research Centre in Cambridge, said: 鈥淭hese findings help us to better understand the experiences of autistic adults, and have wider implications for quality of life.
