Previous research has linked excessive smartphone use to mental health issues such as anxiety, depression, post-traumatic stress disorder (PTSD), attention deficit hyperactivity disorder (ADHD) and problems with self-esteem.

A collaborative team of researchers from the 探花直播 of Chicago, 探花直播 of Cambridge, and the 探花直播 of Minnesota, developed the Health and Addictive Behaviours Survey to assess mental health and well-being in a large sample of university students. They used the survey to investigate the impact of smartphone use on university students. Just over a third (3,425) of students invited to take the test responded. 探花直播results are published today in the Journal of Behavioral Addictions.

探花直播self-report survey consisted of 156 questions. Based on their responses, the students were given a score ranging from 10 to 60, with a score of 32 and above being defined as problematic smartphone use. This definition was based on a threshold recommended previously in clinical validation studies using the scale. Typical characteristics of problematic use include: excessive use; trouble concentrating in class or at work due to smartphone use; feeling fretful or impatient without their smartphone; missing work due to smartphone use; and experiencing physical consequences of excessive use, such as light-headedness or blurred vision.

探花直播researchers found that one in five (20%) of respondents reported problematic smartphone use.

Problematic smartphone use was greater among female rather than male students 鈥 64% of problem users were female. Importantly, the researchers found a link between problematic smartphone use and lower grade point averages (academic achievement).

鈥淎lthough the effect of problematic smartphone use on grade point averages was relatively small, it鈥檚 worth noting that even a small negative impact could have a profound effect on an individual鈥檚 academic achievement and then on their employment opportunities in later life,鈥 said Professor Jon Grant from the Department of Psychiatry & Behavioral Neuroscience at the 探花直播 of Chicago.

While students reporting problematic smartphone use tended be less sexually active than their peers (70.9% compared to 74%), the proportion of students reporting two or more sexual partners in the past 12 months was significantly higher among problem users: 37.4% of sexually-active problematic smartphone users compared with 27.2% sexually-active students who reported no problem use. 探花直播proportion with six or more sexual partners was more than double among sexually-active problematic smartphone users (6.8% compared to 3.0%).

鈥淪martphones can help connect people and help people feel less isolated, and our findings suggest that they may act as an avenue for sexual contact, whether through sustained partnerships or more casual sex,鈥 added Dr Sam Chamberlain, Wellcome Trust Clinical Fellow and Honorary Consultant Psychiatrist from the Department of Psychiatry at the 探花直播 of Cambridge and the Cambridge & Peterborough NHS Foundation Trust. 聽聽

探花直播researchers found that alcohol misuse was significantly higher in those with problematic smartphone use compared to the control group. To assess this, the team used a scale known as the Alcohol Use Disorders Identification Test: a score of eight or above indicates harmful alcohol use. 33.3% of problematic smartphone users scored eight or above compared to 22.5% of other smartphone users. 探花直播researchers found no significant link with any other form of substance abuse or addiction.

In terms of other mental health problems, the researchers found that problematic smartphone use was significantly associated with lower self-esteem, ADHD, depression, anxiety, and PTSD, mirroring similar findings elsewhere.

鈥淚t鈥檚 easy to think of problematic smartphone use as an addiction, but if it was that simple, we would expect it to be associated with a wide range of substance misuse problems, especially in such a large sample, but this does not seem to be the case,鈥 added Dr Sam Chamberlain.聽

鈥淥ne possible explanation for these results is that people develop excessive smartphone use because of other mental health difficulties. For example, people who are socially isolated, those who experience depression or anxiety, or those who have attention problems (as in ADHD) may be more prone to excessive smartphone use, as well as to using alcohol. Smartphone use likely develops earlier in life 鈥 on average 鈥 than alcohol use problems and so it is unlikely that alcohol use itself leads to smartphone use.鈥

While the sample size for this study was relative large, suggesting that the findings should be fairly robust, the researchers point out that as a cross-sectional study (one that takes a 鈥榮napshot鈥 at one particular time, rather than following people over a longer period), and so direction of causality cannot be established. In other words, the study cannot say that problematic smartphone use leads to mental health issues or vice versa.

探花直播researchers point out the effect sizes were also generally small, and that more research is needed into positive and negative effects of smartphone use and mental health, including how this changes over time.

A survey of more than 3,400 university students in the USA has found that one in five respondents reported problematic smartphone use. DBS acts as a brain pacemaker to normalise abnormal brain activity. It is well-tolerated, effective and widely used for neurological disorders and obsessive compulsive disorder.

Although there have been several proof-of-concept studies that suggest DBS is effective in addictions, Brain-PACER 鈥 a collaboration between the 探花直播 of Cambridge, Kings College London and the 探花直播 of Oxford 鈥 is the first major, multicentre study to use DBS to treat craving and relapse in severe addiction.

Chief Investigator Professor Valerie Voon, from the Department of Psychiatry at the 探花直播 of Cambridge, said: 鈥淲hile many people who experience alcohol or drug addiction can, with the right support, control their impulses, for some people, their addiction is so severe that no treatments are effective. Their addiction is hugely harmful to their health and wellbeing, to their relationships and their everyday lives.

鈥淚nitial evidence suggests that deep brain stimulation may be able to help these individuals manage their conditions. We鈥檝e seen how effective it can be for other neurological disorders from Parkinson鈥檚 to OCD to depression. We want to see if it can also transform the lives of people with intractable alcohol and opioid addiction.鈥

探花直播primary aim of the Brain-PACER study is to assess the effects of DBS to treat alcohol and opioid addiction in a randomised controlled trial study. Its mission is twofold: to develop effective treatments for addiction and to understand the brain mechanisms that drive addiction disorders.

DBS is a neurosurgical treatment that involves implanting a slender electrode in the brain and a pacemaker under general anaesthesia. These electrodes deliver electrical impulses to modulate neural activity, which can help alleviate symptoms of various neurological and psychiatric disorders.

Keyoumars Ashkan, Professor of Neurosurgery at King鈥檚 College Hospital and the lead surgeon for the study, said: 鈥淒eep brain stimulation is a powerful surgical technique that can transform lives. It will be a major leap forward if we can show efficacy in this very difficult disease with huge burden to the patients and society.鈥

During surgery, thin electrodes are carefully placed in precise locations of the brain. These locations are chosen based on the condition being treated. For addiction, the electrodes are placed in areas involved in reward, motivation, and decision-making.

Harry Bulstrode, Honorary Consultant Neurosurgeon at Cambridge 探花直播 Hospitals NHS Foundation Trust聽and Clinical Lecturer at the 探花直播 of Cambridge, said: "We see first-hand how deep brain stimulation surgery can be life-changing for patients with movement disorders such as Parkinson鈥檚 disease and essential tremor. Thanks to this trial, I am now hopeful that we can help patients and their families 鈥 who have often struggled for years 鈥 by targeting the parts of the brain linked to addiction."

Dr David Okai, Visiting Senior Lecturer from the Institute of Psychiatry, Psychology & Neuroscience, King鈥檚 College London, added: 鈥淒BS is safe, reversible and adjustable, so it offers a flexible option for managing chronic conditions. We hope it will offer a lifeline to help improve the quality of life for patients whose treatment until now has been unsuccessful.鈥

Details on the trial, including criteria for participation, can be found on the Brain-PACER website.

探花直播research is supported by the Medical Research Council, UK Research & Innovation.

People suffering from severe alcohol and opioid addiction are to be offered a revolutionary new technique involving planting electrodes in the brain to modulate brain activity and cravings and improve self-control.

We鈥檝e seen how effective deep brain stimulation can be for neurological disorders from Parkinson鈥檚 to OCD to depression. While brain development continues into adulthood, grey matter growth peaks before adolescence. 聽

Low grey matter volume in the left side of the ventromedial prefrontal cortex may be an 鈥渋nheritable biomarker鈥 for nicotine addiction, say researchers 鈥 with implications for prevention and treatment.聽

In addition, the scientists found that the opposite, right part of the same brain region also had less grey matter in smokers.

Importantly, loss of grey matter in the right prefrontal cortex appears to speed up only after someone has started smoking. This region is linked to the seeking of sensations.

探花直播team argue that less grey matter in the left forebrain could lower cognitive function and lead to 鈥渄isinhibition鈥: impulsive, rule-breaking behaviour arising from a limited ability to consider consequences. This may increase the chances of smoking at a young age.

Once a nicotine habit takes hold, grey matter in the right frontal lobe shrinks, which may weaken control over smoking by affecting 'hedonic motivation': the way pleasure is sought and managed. Excessive loss of grey matter in the right brain was also linked to binge drinking and marijuana use.聽

Taken together, the findings point to a damaged 鈥渘eurobehavioural mechanism鈥 that can lead to nicotine use starting early and becoming locked into long-term addiction, say researchers. 探花直播study used data from the IMAGEN project and is published in the journal Nature Communications.

鈥淪moking is perhaps the most common addictive behaviour in the world, and a leading cause of adult mortality,鈥 said Prof Trevor Robbins, co-senior author from Cambridge鈥檚 Department of Psychology.

鈥 探花直播initiation of a smoking habit is most likely to occur during adolescence. Any way of detecting an increased chance of this, so we can target interventions, could help save millions of lives.鈥

Annual deaths from cigarettes are expected to reach eight million worldwide by the end of the decade. Currently, one in five adult deaths each year are attributed to smoking in the US alone.

鈥淚n our study, reduced grey matter in the left prefrontal cortex is associated with increased rule-breaking behaviour as well as early smoking experiences. It could be that this rule-breaking leads to the violation of anti-smoking norms,鈥 said Robbins.

Co-author Prof Barbara Sahakian from Cambridge鈥檚 Department of Psychiatry said: 鈥 探花直播ventromedial prefrontal cortex is a key region for dopamine, the brain鈥檚 pleasure chemical. As well as a role in rewarding experiences, dopamine has long been believed to affect self-control.聽

鈥淟ess grey matter across this brain region may limit cognitive function, leading to lower self-control and a propensity for risky behaviour, such as smoking.鈥

探花直播study used data gathered by the IMAGEN project from sites in four European countries: UK, Germany, France and Ireland. 探花直播researchers compared brain imaging data for those who had smoked by age 14 with those who had not, and repeated this for the same participants at ages 19 and 23.

Those with smoking experience by 14 years of age had significantly less grey matter in the left prefrontal cortex, on average. Additionally, those who started smoking by age 19 also had less grey matter in their left prefrontal cortex at 14, indicating a potential causal influence.

探花直播scientists also looked at the right ventromedial prefrontal cortex. Grey matter loss occurs in everyone as they age. However, those who smoked from age 14 as well as those smoking from age 19 both ended up with excessive grey matter loss in the right frontal lobe.

For the right prefrontal cortex, 19-year-old smokers who did not start during adolescence had similar grey matter levels at age 14 to those who never smoked at all. This suggests a rapid reduction in the right ventromedial prefrontal cortex only begins with the onset of smoking.

Data at age 23 showed that grey matter volume in the right prefrontal cortex shrank at a faster pace in those who continued to smoke, suggesting an influence of smoking itself on prefrontal function.

Researchers also analysed data from two questionnaires completed by participants to investigate the personality traits of novelty seeking and sensation seeking.

鈥淏oth questionnaires examine the pursuit of thrilling experiences, but they measure distinct behaviours,鈥 said Robbins. 鈥 探花直播sensation seeking scale focuses on pleasurable experiences, while the novelty seeking questionnaire includes items on impulsiveness and rule-breaking.鈥

Less grey matter in the left prefrontal cortex was associated with novelty seeking, particularly disorderly and rule-breaking behaviour, while reduced grey matter volume in the right prefrontal cortex was linked to sensation seeking only.

Lead author Prof Tianye Jia from Fudan 探花直播 added: 鈥淟ess grey matter in the left frontal lobes is linked to behaviours that increase the likelihood of smoking in adolescence.

鈥淪mokers then experience excessive loss of grey matter in the right frontal lobes, which is linked to behaviours that reinforce substance use. This may provide a causal account of how smoking is initiated in young people, and how it turns into dependence.鈥

Reference:
Jia, Tianye; Xiang, Shintong et al. Association between vmPFC gray matter volume and smoking initiation in adolescents. It can lead to financial, emotional, and relationship problems, including interpersonal violence, and in some cases, engagement in illicit activities to fund their addiction.

Between four and nine people in 1,000 are estimated to experience problem gambling. 探花直播disorder appears to be more common in men compared to women and often develops during adolescence and young adulthood. For reasons that are unclear, some groups appear to be disproportionately affected 鈥 for example, in the UK, some research suggests it is more common among Black British people.

People with gambling disorders have high rates of other mental health conditions, including anxiety and mood disorders, substance abuse disorders, impulse control disorders, and attention-deficit/hyperactivity disorder (ADHD), though these are often undiagnosed. It is also associated with a considerably increased risk of suicide.聽

Professor Barbara Sahakian from the Department of Psychiatry at the 探花直播 of Cambridge said: 鈥淕ambling disorders are a serious problem in the UK yet remain low on the list of priorities in UK healthcare. There鈥檚 a chronic lack of investment and only belated acknowledgment of the problem, which means that the true extent of gambling-related harm and the related resource pressure are ignored or unrecognised.

鈥淲hile in the past, gambling largely took place in establishments such as betting shops or casinos, it has now become much easier to access in secret, particularly on smartphones and computers, so we鈥檙e seeing numbers increase particularly among adolescents and women. In short, it鈥檚 no longer necessary to leave your home to gamble.鈥

In 2020, a group of clinicians and scientists set up the National UK Research Network for Behavioural Addictions (NUK-BA) to identify unmet needs in terms of research and treatment provision for behavioural addictions, including gambling disorder, in the UK. 探花直播organisation does not accept any funding from the gambling industry.

In their article today, experts writing on behalf of NUK-BA say there is 鈥渁 marked lack of dedicated explicit independent funding for research into Gambling Disorder鈥. 探花直播limited funding available now tends to be linked to the gambling industry: at present, funds for gambling research in the UK are collected by a voluntary levy on companies, and these funds are then administered through a specific organisation whose existence is dependent on this industry funding.

Professor Sam Chamberlain from the Faculty of Medicine at the 探花直播 of Southampton said: 鈥淭o many in the research world, funding that鈥檚 linked to the gambling industry 鈥 even when it鈥檚 administered through a third party 鈥 is compromised. It creates potential conflicts of interest and institutional rules that mean many universities, researchers and clinicians in the UK cannot accept funding administered through this route.

鈥淲ith so little understood about gambling disorder, we clearly need much more research if we鈥檙e going to help those affected. For the purposes of transparency and to ensure that there are no conflicts of interest, this research needs be funded by independent bodies, such as the government or independent charities, and not by the gambling industry.鈥

探花直播NUK-BA has called for the government to implement a 1% statutory levy on industry earnings and says this should be administered by an independent research body unrelated to the gambling industry, such as the Medical Research Council, part of UK Research & Innovation, a non-departmental government body.

In their article, the NUK-BA sets out its top priorities for UK research into gambling disorder:

Conduct independent longitudinal research on prevalence of gambling disorder and at-risk gambling, and gambling harms, including in vulnerable and minority groups.

Select and refine the optimal pragmatic measurement tools.

Identify predictors 鈥 vulnerability and resilience markers 鈥 of disordered gambling in people who gamble recreationally, including in vulnerable and minority groups, longitudinally.

Conduct randomised controlled trials on psychological interventions and drug treatments for gambling disorder.

Optimise our understanding of the neurobiological basis of gambling disorder, including genetics, impulsivity and compulsivity, and other biomarkers.

Develop clinical guidelines based upon the best possible contemporary research evidence to guide effective clinical interventions.

探花直播researchers argue that our understanding of the prevalence and course of gambling disorder is further limited by a lack of appropriate expert consultation, noting that despite the Gambling Commission stating in its recent consultation that experts had been consulted, not a single member of NUK-BA group reported having been consulted.

Professor Henrietta Bowden-Jones, from the National Problem Gambling Clinic & National Centre for Gaming Disorders and the 探花直播 of Cambridge, said: 鈥 探花直播UK is fortunate to have world leading experts in the study of addiction as well as behavioural addiction. This places us in an excellent position to further our understanding and to develop new treatments. With sufficient independent funding, we will be able to make great progress in preventing problem gambling and improving the quality of life and wellbeing of those recovering from behavioural addictions and their families.鈥

Too little research is being conducted into problem gambling 鈥 and what research there is, is often funded by the gambling industry rather than by independent means, say a team of experts. It is during this time that individuals begin to demonstrate behaviours that are associated with addiction and which suggest that they may be at risk.

One of these behaviours is impulsivity. Sometimes, we need to make quick decisions, for example in response to a danger or a threat. At other times, it is better to hesitate and decide only after careful deliberation. Impulsivity refers to where we respond and act prematurely, without considering the consequences of our actions. While most people occasionally act impulsively, people affected by disorders including attention deficit hyperactivity disorder (ADHD), substance and behavioural addictions, and mental health problems such as depression and anxiety, show much greater levels of impulsivity.

In a study published today in the journal Neuropsychopharmacology, a team of researchers at Cambridge鈥檚 Department of Psychiatry, in collaboration with a group at Aarhus 探花直播 in Denmark, has shown a strong association between increased behavioural impulsivity in young adults and abnormalities in nerve cells in the putamen, a key brain region involved in addictive disorders.

As part of the study, 99 young adults aged 16 to 26 carried out a computer-based measure of impulsivity. 探花直播researchers also scanned the volunteers鈥 brains using a sequence that is sensitive to myelin content. Myelin is a protein-rich sheath that coats the axis of a nerve cell, analogous to the plastic coating that surrounds electrical wiring, and is essential to fast nerve conduction in the brain and body.

探花直播team found that those young adults who displayed higher measures of behavioural impulsivity also had lower levels of myelin in the putamen. This work builds on similar findings in rodent models of impulsivity from scientists at Cambridge and elsewhere.

鈥淧eople who show heightened impulsivity are more likely to experience a number of mental health issues, including substance and behavioural addictions, eating disorders, and ADHD,鈥 says Dr Camilla Nord of the MRC Cognition and Brain Sciences Unit, lead author on the study.

This suggests that impulsivity is an 鈥榚ndophenotype鈥, say the researchers; in other words, a set of behavioural and brain changes that increases people鈥檚 general risk for developing a group of psychiatric and neurological disorders.

鈥淲e know that most mental health symptoms are not specific to particular disorders,鈥 says Dr Nord. 鈥淭his work provides an important piece of the puzzle in establishing brain signatures that are general across a number of mental health disorders, rather than specific to any single one.鈥澛

探花直播putamen is a key brain hub in addiction, sending dopamine signals elsewhere in the brain, and helping mediate how impulsively we behave. 鈥 探花直播significance of decreased myelination implies there are tiny microstructural changes in this part of the brain affecting its function, and thereby affecting impulsivity,鈥 says senior author Dr Valerie Voon, also from Cambridge.

鈥 探花直播degree of myelination alters the speed and efficiency of neuronal communication, meaning that if a population has decreased myelination only in one particular region, as we show, there is something highly local about any changes in neural speed and efficiency,鈥 add co-author Dr Seung-Goo Kim. 聽

Although it is not possible to say definitely whether the decreased myelination causes individuals to behave impulsively, the fact that all participants were healthy and had not been diagnosed with addiction or any other psychiatric diagnosis suggests a more causal link than has been demonstrated in previous studies.

In future, the finding may help in predicting an individual鈥檚 risk of developing a problem with addiction, say the researchers, but they caution that this would require further research and testing.

探花直播research was funded by the Aarhus 探花直播 Research Foundation, the Danish Ministry for Social Affairs and the Interior and the UK Medical Research Council. 探花直播work was also supported by NIHR Cambridge Biomedical Research Centre.

Reference
Nord CL, Kim S, R酶mer Thomsen,K, Callesen MB, Kvamme TL, Jensen M, Pedersen MU, Voon V. 探花直播myeloarchictecture of impulsivity: premature responding in youth is associated with decreased myelination of ventral putamen. Neuropsychopharmacology; I hope to run these sorts of early-stage studies in the next few years, before taking these studies forward into clinical trials in the future, in collaboration with the NHS and other large-scale organisations.鈥

Ultimately, she hopes this will lead to is a future where it is possible to tailor mental health treatments to each patient.

Camilla鈥檚 research involves working with volunteers on a daily basis for computer-based experiments at the MRC unit, or at Addenbrooke鈥檚 for brain stimulation experiments.

鈥淲hen I am not running experiments, I am analysing data, including brain scans, using statistical models to test my hypotheses, or writing papers and giving talks to communicate my findings to the rest of the field and the public.鈥

It is the public engagement side of her work that can be particularly insightful. 鈥淚 think some of the most interesting opportunities I鈥檝e had were when I鈥檝e been given the chance to discuss my work with patients, hear their feedback, and listen to their ideas about what might be most fruitful for our future directions of research. I think this sort of research-patient interaction will be vital to developing better ways of treating mental health difficulties in future.鈥

探花直播text in this work is licensed under a Creative Commons Attribution 4.0 International License. A report last year by the UK government鈥檚 Advisory Council on the Misuse of Drugs found that almost one in 10 of all 16-to 59-year-olds have used cocaine in their lifetime. Cocaine use was implicated in, but not necessarily the cause of 234 deaths in Scotland, England and Wales in 2013. However, despite significant advances in our understanding of the biology of addiction 鈥 including how the brains of people addicted to cocaine may differ in structure 鈥 there is currently no medical treatment for cocaine addiction; most individuals are treated with talking or cognitive therapies.

A team of researchers led by Dr Karen Ersche from the Department of Psychiatry at Cambridge examined the brains of 44 people who were addicted to cocaine and 44 healthy control volunteers. In the cocaine group, they detected excessive amounts of iron in a region of the brain known as the globus pallidus, which ordinarily acts as a 鈥榖rake鈥 for inhibiting behaviour.

Particularly striking was the fact that the concentration of iron in this area was directly linked with the duration of cocaine use 鈥 in other words, the longer that participants had used cocaine, the greater the accumulation of iron. At the same time, the increased iron concentration in the brain was accompanied by mild iron deficiency in the rest of the body, suggesting that iron regulation in general is disrupted in people with cocaine addiction.

鈥淕iven the important role that iron plays in both health and disease, iron metabolism is normally tightly regulated,鈥 explains Dr Karen Ersche from the Department of Psychiatry. 鈥淟ong-term cocaine use, however, seems to disrupt this regulation, which may cause significant harm.

鈥淚ron is used to produce red blood cells, which help store and carry oxygen in the blood. So, iron deficiency in the blood means that organs and tissues may not get as much oxygen as they need. On the other hand, we know that excessive iron in the brain is associated with cell death, which is what we frequently see in neurodegenerative diseases.鈥

探花直播Cambridge researchers now aim to identify the precise mechanisms by which cocaine interacts with iron regulation. Dr Ersche believes the most likely mechanism is that cocaine disrupts iron metabolism, possibly by reducing the absorption of iron from food, increasing the permeability of the blood-brain-barrier so that more iron enters the brain, where it can accumulate.

Although excess iron in the brain is associated with neurodegeneration, there is no suggestion that cocaine addiction leads to an increased risk of Alzheimer鈥檚 or Parkinson鈥檚 disease. 探花直播mechanism underlying the increase in iron in the brain in Parkinson鈥檚 disease, for example, is different to that in cocaine addiction, as are the affected brain regions.

As an essential micronutrient, iron can only be obtained through our diet and cannot be excreted, other than through blood loss. 探花直播researchers now want to find out whether means of correcting the disruptions in iron metabolism might slow down or even reverse the accumulation of iron in the brain, and ultimately help affected individuals to successfully recover from cocaine addiction.

This work was funded by the Medical Research Council and was conducted at the NIHR Cambridge Biomedical Research Centre and the Behavioural and Clinical Neuroscience Institute.

Cocaine addiction may affect how the body processes iron, leading to a build-up of the mineral in the brain, according to new research from the 探花直播 of Cambridge. 探花直播study, published today in Translational Psychiatry, raises hopes that there may be a biomarker 鈥 a biological measure of addiction 鈥 that could be used as a target for future treatments.

Given the important role that iron plays in both health and disease, iron metabolism is normally tightly regulated.
