An estimated 1.9 million people in the UK alone (2.9% of the population) were experiencing self-reported Long COVID as of March 2023, <a href="https://www.ons.gov.uk/peoplepopulationandcommunity/healthandsocialcare/conditionsanddiseases/bulletins/prevalenceofongoingsymptomsfollowingcoronaviruscovid19infectionintheuk/30march2023">according to the ONS</a>. Fatigue remains by far the most common and debilitating symptom and patients are still waiting for an effective treatment.</p> <p> 探花直播study shows that initial infection with SARS-CoV-2 triggers production of the antiviral protein IFN-<em>纬</em>, which is a normal reaction from the immune system. For most people, when their infection clears, COVID-19 symptoms cease and production of this protein stops, but the researchers found that high levels of IFN-<em>纬</em> persisted in some Long COVID patients for up to 31 months.</p> <p>鈥淲e have found a potential mechanism underlying Long COVID which could represent a biomarker 鈥 that is, a tell-tale signature of the condition. We hope that this could help to pave the way to develop therapies and give some patients a firm diagnosis,鈥 said co-author, Dr Benjamin Krishna, from the Cambridge Institute of Therapeutic Immunology &amp; Infectious Disease (CITIID).</p> <p> 探花直播research began in 2020 when Dr Nyarie Sithole (Hughes Hall) set up a Long COVID clinic in Cambridge鈥檚 Addenbrooke鈥檚 Hospital, where he started collecting blood samples from patients and set about studying their immunology. Sithole soon enlisted the support of Dr Benjamin Krishna and Dr Mark Wills from the 探花直播 of Cambridge鈥檚 Department聽of Medicine.</p> <p>鈥淲hen the clinic started, a lot of people didn't even believe Long COVID was real,鈥 Dr Sithole said. 鈥淲e are indebted to all the patients who volunteered for this study, without whose support and participation we would obviously not have accomplished this study鈥.</p> <p> 探花直播team studied 111 COVID-confirmed patients admitted to Addenbrooke鈥檚 Hospital CUH, Royal Papworth Hospital and Cambridge and Peterborough NHS Foundation Trusts at 28 days, 90 days and 180 days following symptom onset. Between August 2020 and July 2021, they recruited 55 Long COVID patients 鈥 all experiencing severe symptoms at least 5 months after acute COVID-19 鈥 attending the Long COVID clinic at Addenbrooke鈥檚.</p> <p> 探花直播researchers analysed blood samples for signs of cytokines, small proteins crucial to the functioning of immune system cells and blood cells. They found that the white blood cells of individuals infected with SARS-CoV-2 produced IFN-<em>纬</em>, a pro inflammatory molecule, and that this persisted in Long COVID patients.</p> <p>Dr Krishna said: 鈥淚nterferon gamma can be used to treat viral infections such as hepatitis C but it causes symptoms including fatigue, fever, headache, aching muscles and depression. These symptoms are all too familiar to Long COVID patients. For us, that was another smoking gun.鈥</p> <p>By conducting 鈥榗ell depletion assays鈥, the team managed to identify the precise cell types responsible for producing IFN-<em>纬</em>. They pinpointed immune cells known as CD8+ T cells but found that they required contact with another immune cell type: CD14+ monocytes.</p> <p>Previous studies have identified IFN-<em>纬</em> signatures using different approaches and cohorts, but this study鈥檚 focus on fatigue revealed a much stronger influence. Also, while previous studies have noticed IFN-y levels rising, they have not followed patients long enough to observe when they might drop back down.</p> <p> 探花直播Cambridge team followed its Long COVID cohort for up to 31 months post-infection. During this follow up period, over 60% of patients experienced resolution of some, if not all, of their symptoms which coincided with a drop in IFN-<em>纬</em>.</p> <h3>Vaccination helping Long COVID patients</h3> <p> 探花直播team measured IFN-<em>纬</em> release in Long COVID patients before and after vaccination and found a significant decrease in IFN-<em>纬</em> post vaccination in patients whose symptoms resolved.</p> <p>鈥淚f SARS-CoV-2 continues to persist in people with Long COVID, triggering an IFN-<em>纬</em> response, then vaccination may be helping to clear this. But we still need to find effective therapies,鈥 Dr Krishna said.</p> <p>鈥 探花直播number of people with Long COVID is gradually falling, and vaccination seems to be playing a significant role in that. But new cases are still cropping up, and then there is the big question of what happens when the next coronavirus pandemic comes along. We could face another wave of Long COVID. Understanding what causes Long COVID now could give us a crucial head start.鈥</p> <h3>Microclotting</h3> <p>Some well-publicised previous studies have proposed microclotting as a principle cause of Long COVID. While not ruling out a role of some kind, these new findings suggest that microclotting cannot be the only or the most significant cause.</p> <h3>Classifying Long COVID</h3> <p>This study argues that the presence of IFN-<em>纬</em> could be used to classify Long COVID into subtypes which could be used to personalise treatment.聽</p> <p>鈥淚t鈥檚 unlikely that all the different Long COVID symptoms are caused by the same thing. We need to differentiate between people and tailor treatments. Some patients are slowly recovering and there are those who are stuck in a cycle of fatigue for years on end. In the UK, according to the Office for Health Improvement &amp; Disparities, almost two-thirds of adults are overweight or living with obesity. This increases an individual鈥檚 risk of developing a number of health problems, including type 2 diabetes, heart disease and stroke, cancer and poorer mental health.</p>&#13; &#13; <p>A large number of factors influence how much we eat and the types of food we eat, including our genetics, hormone regulation, and the environment in which we live. What happens in our brains to tell us that we are hungry or full is not entirely clear, though studies have shown that the hypothalamus, a small region of the brain about the size of an almond, plays an important role.</p>&#13; &#13; <p>Dr Stephanie Brown from the Department of Psychiatry and Lucy Cavendish College, 探花直播 of Cambridge, said: 鈥淎lthough we know the hypothalamus is important for determining how much we eat, we actually have very little direct information about this brain region in living humans. That鈥檚 because it is very small and hard to make out on traditional MRI brain scans.鈥</p>&#13; &#13; <p> 探花直播majority of evidence for the role of the hypothalamus in appetite regulation comes from animal studies. These show that there are complex interacting pathways within the hypothalamus, with different cell populations acting together to tell us when we are hungry or full.</p>&#13; &#13; <p>To get around this, Dr Brown and colleagues used an algorithm developed using machine learning to analyse MRI brain scans taken from 1,351 young adults across a range of BMI scores, looking for differences in the hypothalamus when comparing individuals who are underweight, healthy weight, overweight and living with obesity.</p>&#13; &#13; <p>In a study published today in <em>Neuroimage: Clinical</em>, the team found that the overall volume of the hypothalamus was significantly larger in the overweight and obese groups of young adults. In fact, the team found a significant relationship between volume of the hypothalamus and body-mass index (BMI).</p>&#13; &#13; <p>These volume differences were most apparent in those sub-regions of the hypothalamus that control appetite through the release of hormones to balance hunger and fullness.</p>&#13; &#13; <p>While the precise significance of the finding is unclear 鈥 including whether the structural changes are a cause or a consequence of the changes in body weight 鈥 one possibility is that the change relates to inflammation. Previous animal studies have shown that a high fat diet can cause inflammation of the hypothalamus, which in turn prompts insulin resistance and obesity. In mice, just three days of a fat-rich diet is enough to cause this inflammation. Other studies have shown that this inflammation can raise the threshold at which animals are full 鈥 in other words, they have to eat more food than usual to feel full.</p>&#13; &#13; <p>Dr Brown, the study鈥檚 first author, added: 鈥淚f what we see in mice is the case in people, then eating a high-fat diet could trigger inflammation of our appetite control centre. Over time, this would change our ability to tell when we鈥檝e eaten enough and to how our body processes blood sugar, leading us to put on weight.鈥</p>&#13; &#13; <p>Inflammation may explain why the hypothalamus is larger in these individuals, the team say. One suggestion is that the body reacts to inflammation by increasing the size of the brain鈥檚 specialist immune cells, known as glia.</p>&#13; &#13; <p>Professor Paul Fletcher, the study鈥檚 senior author, from the Department of Psychiatry and Clare College, Cambridge, said: 鈥 探花直播last two decades have given us important insights about appetite control and how it may be altered in obesity. Metabolic researchers at Cambridge have played a leading role in this.</p>&#13; &#13; <p>鈥淥ur hope is that by taking this new approach to analysing brain scans in large datasets, we can further extend this work into humans, ultimately relating these subtle structural brain findings to changes in appetite and eating and generating a more comprehensive understanding of obesity.鈥</p>&#13; &#13; <p> 探花直播team say more research is needed to confirm whether increased volume in the hypothalamus is a result of being overweight or whether people with larger hypothalami are predisposed to eat more in the first place. It could, in the future, help direct the development of new treatment approaches for those living with HIV.</p>&#13; &#13; <p>HIV remains a major threat to global health. According to UNAIDS, there were 38.4 million people living with HIV globally in 2021. A combination of pre-exposure drugs and medicines that dramatically reduce viral loads has had a major impact on transmission, yet 1.5 million people were newly infected in 2021. And while treatments have improved dramatically since the virus was first identified, 650,000 people still died from AIDS-related illnesses in that year.</p>&#13; &#13; <p>Viral load is the amount of a virus in a patient鈥檚 system. Higher levels are known to correlate with faster disease progression and increased risk of transmission. But viral load varies widely among infected individuals, influenced by a number of factors including an individual鈥檚 genetic makeup.</p>&#13; &#13; <p>Most of what we know about the relationship between our DNA and HIV comes from studies among European populations. But given that HIV disproportionately affects people on the African continent 鈥 more than 25 million people who are HIV-positive live on the continent 鈥 it鈥檚 important to better understand the role of genetics in HIV infection in African populations.</p>&#13; &#13; <p>To investigate this question, researchers analysed the DNA of almost 4,000 people of African ancestry living with HIV-1, the most common type of the virus. They identified a variant within a region on chromosome 1 containing the gene CHD1L which was associated with reduced viral load in carriers of the variant. Between 4% and 13 % of people of African origin are thought to carry this particular variant.</p>&#13; &#13; <p>Paul McLaren from the Public Health Agency of Canada鈥檚 National Microbiology Laboratory, joint first author, said: 鈥淎frican populations are still drastically underrepresented in human DNA studies, despite experiencing the highest burden of HIV infection. By studying a large sample of people of African ancestry, we鈥檝e been able to identify a new genetic variant that only exists in this population and which is linked to lower HIV viral loads.鈥</p>&#13; &#13; <p>CHD1L is known to play a role in repairing damaged DNA, though it is not clear why the variant should be important in reducing viral load. However, as HIV attacks immune cells, researchers at the 探花直播 of Cambridge鈥檚 Department of Medicine, led by Dr Harriet Groom and Professor Andrew Lever, used stem cells to generate variants of cells that HIV can infect in which CHD1L had either been switched off or its activity turned down.</p>&#13; &#13; <p>HIV turned out to replicate better in a type of immune cell known as a macrophage when CHD1L was switched off. In another cell type, the T cell, there was no effect 鈥 perhaps surprising since most HIV replication occurs in the latter cell type.</p>&#13; &#13; <p>Dr Groom said: 鈥淭his gene seems to be important to controlling viral load in people of African ancestry.
